I fully agree and volunteer to give Glamour Esthetics my permission to post my photos/videos with the possibility of it being posted as marketing and advertisement content. This does includes before & after pictures, progress pictures, etc.
Agreement
By signing this consent form, I have read and acknowledge that:
- I have given my esthetician my verbal consent to the content she makes with my photos/videos.
- I have voiced to my esthetician all of my boundaries that I am comfortable and uncomfortable with, and she is well aware of them. All content that cross one of these boundaries shall be removed off of everything, immediately and indefinitely.
I also understand and declare that;
- If I did consent, it is freely given as a public service to Glamour Esthetics, without expecting payment.
- I hereby agree to waive my rights to any claims to whatever Glamour Esthetics may use of my images.
- I cannot threaten or pursue legal action against my Esthetcian if I fail to meet any of the standards listed above.
- If I did not consent, I will not have to worry about anything above. I am also free to change my mind and will have to create a new form should I choose to start sharing my photos.
By submitting this form with my signature below, I am affirming to the statements mentioned above and the effectivity of this agreement shall commence.
