The information I have provided in this application is true and correct. I understand that completion of a Volunteer Waiver and a Foster/Adoption Contract will also be required if this application is approved. I understand that any misrepresentation of facts in this application or failure to comply with any requirements of LCHS I PAWS, (LCHS I PAWS), including but not limited to NOT fostering/adopting for other rescues/agencies while fostering for LCHS I PAWS, will result in removal of LCHS I PAWS foster animal(s) from my home. I agree to immediately surrender physical custody of the LCHS I PAWS foster animal(s) should I be found negligent of the terms and conditions set forth in the Foster/Adoption Contract.
