This tool is to guide centers in assessing center-level outcomes, QI skills, Project Management, and ICN network activities typically needed to conduct reliable Pre-Visit Planning and Population Management. PVP & PM skills and activities build on foundational ICN processes that reliably: identify and maintain IBD patient list; update registry population tracker; register all patients into Outcomes Registry; and accurately capture visit data.
Centers are not expected to accomplish these skills at once. This is a tool to assess and guide development of PVP and PM processes. GI centers can refer to Foundations, PVP, and PM Key Driver Diagrams, and participate in monthly Learning Labs while working to reliably establish these processes and problem-solve challenges.
