I, the undersigned parent or legal guardian of the above-named child, hereby consent to my child’s participation in the online breathing techniques instruction class. I understand that this class will be conducted over an online platform and will involve learning and practicing various breathing techniques.

Waiver and Release of Liability:

I acknowledge that participation in any physical activity, including breathing techniques, carries certain inherent risks. I hereby release and discharge Yapah Judah, Breathing Techniques for Children from Yapah Life™, and any affiliated organizations, their staff, instructors, and agents, from any and all liability, claims, demands, or causes of action that may arise from my child's participation in this class, whether caused by negligence or otherwise.

I understand that the breathing techniques taught in this class are not a substitute for medical advice, diagnosis, or treatment. I affirm that my child is in good health and physically capable of participating in this class. If my child experiences any discomfort, pain, or adverse symptoms, I agree to seek appropriate medical advice and discontinue participation until it is deemed safe by a medical professional.

Photography and Media Release:

I give my consent for photographs, videos, or other media recordings of my child during the class to be used for educational or promotional purposes by Yapah Judah, Breathing Techniques for Children from Yapah Life™. I understand that my child's identity will be protected and that no personal information will be shared.

Confidentiality and Privacy:

I understand that my child’s personal information and participation details will be kept confidential and will not be shared with any third parties without my explicit consent, except as required by law.

Acknowledgment of Understanding:

I have read this waiver and fully understand its terms. I acknowledge that I am signing it freely and voluntarily and intend my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
