As a reminder, since permanent jewelry is a custom piece, it is important to note that the responsibility for its care, maintenance, and any potential adjustments rests with the parent or guardian. So Golden LLC is not responsible for any lost, stolen, or broken pieces.

Although every precaution will be made to ensure your safety and well-being before, during and after the permanent jewelry application, please be aware of the possible risks below:

So Golden LLC (SG) is pleased to offer permanent jewelry welding. I understand and agree to the following: (1) the permanent jewelry item(s) are final sale when the chain is cut and SG begins to perform the Service; (2) there are no refunds on jewelry items or any other products;(3) except as provided herein, SG does not offer replacements, refunds, or discounts to replace or repair lost, broken, or damaged any products, and (4) except as expressly stated in purchase receipt. SG makes no representation or warranties concerning the Service or with respect to the purchased permanent jewelry item and disclaims any and all express and implied warranties. SG technicians will check the jump ring for proper welding before you leave the chair and the appoint for the Service is completed. SG offers complementary re-welding for any permanent jewelry item that breaks at the weld, but will not provide the Service or any re-welding if the permanent jewelry chain breaks anywhere other than at the site of the weld. In performing the Service, SG technicians take reasonable precautions during and after the Service to ensure everyone's safety, such as using protective cloth over the skin and under the chain, making available safety glasses for eye protection, and providing an option to watch the welding process through a smartphone. However, SG has advised me of and I understand that there are nevertheless inherent risks associated with receiving the Service, including but not limited exposure to UV light, hazards to eyes, skins, and other body parts, dangers of accidents, sickness, allergic reactions, serious and permanent personal and bodily injury, including death, and that by requesting SG to perform the Service, I agree to assumes all risks associated with the Service. Further, in consideration for SG providing the Service, I, on behalf of myself, heirs, and assigns, hereby release SG and its members, employees, and agents from and against any and all claims, damages, lawsuits, and liability of any kind related to personal injury, sickness, allergic reactions, death, or damage to tangible property arising as a consequence of the Service, failure to follow any safety recommendations by technicians for the Service, or my use of the permanent jewelry item, regardless of the cause and of the negligence of GKC Parties. GKC is not responsible for lost or stolen chains, nor will were place lost or stolen chains. SG is not responsible for any allergies, or rashes caused by jewelry. All Sales are final. No refunds will be given under any circumstances as the jewelry is cut to your preference, and we are unable to ever use again. All chains need to stay clear away from chemicals. SG is not responsible for chemical damage of any chains. Certain chemicals can cause rapid wear, tear, or tarnish of chains. No exchanges or refunds will be given for chemical damage. SG as the right to refuse service. As stated, all sales are final. No refunds, or exchanges. Do you agree to the terms, and understand all of the above?

I understand that SG may take photographs or record videos of the Service, in particular, if the Service is being performed at private parties or public pop-up events for use within the SG portfolio. The photographs and recordings will be used for marketing and promotional purposes. I am irrevocably authorizing SG to use, reproduce, edit, and display photographs and recordings, which may contain my image, likeness, appearance, and voice as they appear in the image or recording, in any medium or format whatsoever (including but not limited to social media sites, websites, marketing materials and emails, print publications, etc.) and on any platform for advertising and promotional purposes. In the event that one or more provisions of this Acknowledgement, Informed Consent, and Release becomes or is determined by a court of competent jurisdiction or arbitrator to be invalid, illegal or unenforceable in any respect, then the enforceability of the remaining provisions contained herein shall not be affected thereby and shall remain in full force and effect and any such provision that is determined to be unenforceable shall be reformed to make such provision valid and enforceable in a manner that provides Adorn the maximum protections permitted by law.
