  • UCAC 2025-2026 Cotillion Season Application

    Please review the guidelines below. The application can be accessed by utilizing the 'NEXT' button.
    • Applicants must be current sophomores, juniors, or seniors in high school with a minimum 2.5 GPA. An official transcript showing GPA and bearing the school seal is required. Debutantes should not be or become parents during the cotillion season. They should not have a record of disciplinary action within the last two years.

    • The applicant must be in good standing with both her school and community. She must submit recommendation forms from a school official and a community member. Recommenders will complete the form which will be submitted directly to us.

    • A non-refundable fee of $175 is required. Once you submit your completed application, instructions on how to pay will be provided. Please note that bank and credit card payments will incur a processing fee. Money orders and cashier's checks do not.

    • The completed application, including the official transcript, recommendation forms, and $175 fee, must be submitted by Friday, August 29, 2025.

    • Debutantes must attend at least 85% of scheduled activities scheduled. A calendar of events will be provided for your convenience. The cotillion season runs fall to spring. Most activities are scheduled on Sundays.

    • Debutantes will engage in group community service projects and complete additional hours independently.

    • The debutante, her Jr. Marshal, and her parent/guardian must arrange their own transportation to all activities.

    • Debutante and Jr. Marshal parents/guardians must complete all required paperwork for event permissions.

    • Union County Alumnae will adhere to Delta Sigma Theta Sorority, Inc. COVID safety protocols.

    Important Dates to Remember

    08/29/2025: Completed application with all supporting documents due by 11:59pm

    09/07/2025: Orientation Session for Debutantes, Matrons, and Marshals from 3:00–5:30PM location TBD (Jr. Marshals are required to attend this orientation if they are 18 years old or older). There is a separate training/orientation for Jr. Marshals and their parents that will be scheduled at a later date. 

    09/19/2025: Jr. Marshal forms due by 11:59pm

    All application materials are to be submitted by the August 29, 2025 due date. Late submissions will not be considered.

  • Please note that a recommendation form from a school official and a member of the community are required to complete your application. Once you complete this section, we will send an email with instructions to your recommenders. Forms can also be found on the chapter website. Forms will be completed via Jotform and submitted directly to us by the recommender. Recommenders MUST use the forms provided. No emailed or mailed recommendation letters will be accepted. Please request a printed copy to verify submission. All forms must be submitted on or before 11:59pm on Friday, August 29, 2025 for your application to be complete.

  • Parent/Guardian's Information

  • An active email address is required for the applicant AND the parent(s); you will be notified by email if accepted into the program no later than Wednesday, September 3, 2025. The debutante, matron (mother/mother-figure), and marshal (father/father-figure) should make plans to attend the orientation session on September 7, 2025. If your Jr. Marshal is 18 years old or older, they will need to attend this session as well.

  • Risk Management and Activity Fee Payment Information

    Thank you for considering participation in one of our Delta Sigma Theta Sorority, Inc. youth programs. Our programs follow strict policies and procedures designed to ensure the safety of youth participants and minimize any risks associated with the program.

    All parents and adults involved in the Cotillion must complete and pay for a background check. Additionally, any debutante or Jr. Marshal aged 18 or older must also undergo a background check.

    The designated vendor for the background check will be provided at the Cotillion orientation. It is essential to use this vendor to receive clearance from our Risk Management Team. Fees range from $30 and up per person over age 18, depending on the states required.

    Delta's foremost priority is the safety and well-being of all participants in our programs. Adherence to the policies outlined in the sorority's Risk Management Manual is crucial to achieving this goal. Further details will be provided during the orientation session.

  • Please note that your application is not complete until we receive this completed form, the $175 activity fee, your school personnel recommendation form and community recommendation form.

    The $175 activity fee will be returned to ineligible applicants; however any processing fees will not. Once accepted to be a debutante, the $175 activity fee is non-refundable.

    Applications must be completed by August 29, 2025 via this form. Once you hit submit, you will be redirected to a separate form to choose payment option for the $175 payment fee. Depending on your mode of payment, there may be an additional processing fee.

By signing this application, I acknowledge that I have truthfully completed it and understand that I may be contacted if there are any questions or concerns.

    I authorize the Cotillion Lead, Union County Alumnae Chapter President, Union County Alumnae Vice Presidents, or their designee to verify any information provided in this application.

    I understand that falsification of any information will result in my dismissal from the Cotillion program, and any money received from me or on my behalf will not be refunded. I also understand that if I voluntarily withdraw from the Cotillion program, any money received from me or on my behalf will not be refunded.

    I understand that it is my responsibility to find a junior marshal who will escort me during the cotillion season by September 19, 2025. The link for his application can be found at www.unioncountydst.org/cotillion beginning June 13, 2025. He will be required to attend designated activities, all designated dance rehearsals and participate in the culminating ball. If he is 18 years old or older, he MUST attend risk management training on September 7, 2025 during orientation.

    I understand that my daughter will need to have a marshal (father or father figure) and a junior marshal (responsible young male) participate during the cotillion season. I agree that I, and any other adults aged 18 and older who participate in the Cotillion program on behalf of my daughter, will need to pay for and complete a background check as required. There will also be a risk management training and agreement that we must complete. I recognize that this is necessary to ensure the safety of my child and the other participants.
