I also understand that in the event medical intervention is necessary, every attempt will be made to contact immediately the persons listed on this form. If I cannot be reached in an emergency during Religious Education, I give my permission to the physician selected by the activity leader to hospitalize in order to secure medical treatment and/or an injection, anesthesia, or surgery for my child(ren) as deemed necessary.

Tuition Fees

1 Student = $85, 2 Students = $145, 3+ Students = $175

Non-registered family Fee = $40 | Late Registration Fee (After Sept. 1st) = $25

1st Confession Retreat = $10 per student, 1st Communion Retreat = $10 per student, Confirmation Year 1 & 2 Retreat = $150 per student
