Return to Duty Step 5 Only

Once you complete this form and make your payment or payment arrangements, you will get your testing pass by email and you can search for locations by the link in the email. Once you take your test, it takes about 2-3 days to get the Negative test results. Then they are reported directly to the Clearinghouse and you will immediately be out of prohibited status. *Price includes cost of test.

****NEW UPDATES!!*** Your FolIow up testing date cannot be entered within 180 days of your Return to Duty test date. Meaning if your RTD date was 01/01/2025 you cannot have the Follow up Completion Date no sooner than 06/01/2025. I will contact your SAP personally and get your follow up plan and we will go by the plan provided by your SAP. Minimum number is 6 follow ups within a year per FMCSA. Each follow up test is $90.00 per test. Once your plan is completed, I report the date to Clearinghouse and you will have a closed return to duty process on the Clearinghouse.

REPORTING Another RTD Test

If you have a current return to duty test result that someone refused to enter for you an employer doesn't know how to enter, attach a picture and email to returntodutynow@yahoo.com and I will report it to the Clearinghouse for you.

SAP Re-Evaluation/Follow up Plan

If your SAP is no longer reachable, you cannot get the follow up plan from any source, your SAP passed away (it happens!), your previous employer went out of business and took the follow up plan with them, then you can have a re-evaluation completed and a new follow up plan can be created for you so that you can move forward with the follow up plan and getting that completed. YOU MUST BE AT LEAST ON STEP 4 and the Determine Eligible for RTD test date has to be in the Clearinghouse already for this to be an option for you. *********** Side note here: If you are trying to change your SAP you will not be able to do so unless you call FMCSA and make the request -- normally if your SAP is responding to FMCSA and they have already entered the Initial Assessment Date (Step 3) then you will not normally see FMCSA remove a SAP.

SAP Assessment

You will complete steps 1-4 with our in house SAP who is able to conduct the DOT Assessments and has credentials to do so. Your evaluation will be conducted electronically and if any type of class is recommended they are one time 25.00 online. *This can vary as far as the SAP may recommend a class and sometimes not. Once evaluated, you are ready for a return to duty (step 5)
