FACILITY USE AGREEMENT

This agreement is made between the person hosting this event and Waldron Public Schools, concerning the use of a WPS facility.

The HOST agrees to comply with the following terms and conditions:

1. The HOST will pay the amount $50.00 as rent for the use of the facilities.

2. The HOST will also submit a damage/cleaning deposit in the amount of $50.00. This damage/cleaning deposit will be returned to the HOST if there are no damages to the facilities or the equipment and if facilities are clean. If there are damages caused to the facilities, the HOST will reimburse WPS for damages.

3. The HOST assumes all responsibility and liability for the requested use and agrees to defend, indeminify and hold WPS harmless from any and all loss, damage or injury to any person or property arising from use of the facilities.

4. The HOST agrees to clean facilities after use and remove all trash. If HOST fails to do so, HOST will reimburse WPS for the cost of cleanup.

5. Employees of WPS will not have to pay a rental fee if facilites are left clean, free of trash, and no damages have been made to facilites.

6. The HOST will be solely responsibile to provide adequate supervision of the activiites conducted in WPS facilities.

7. The HOST will comply with all applicable state laws and WPS polices.
