YOUTH LEADER:

Please make sure each person in your group (sponsors included) submits a waiver of liability. Both a printable version AND an online version are available on our website under the Resources tab.

If you are using the online form, give the teens/parents your email (or whatever email would be best) and tell them to enter it in the "Church Email" box. All confirmation emails will then be sent to that email.
