  • Memorandum of Understanding and Terms and Conditions for Repatriation of Mortal Remains

  • This document outlines the responsibilities of Agnee Funeral Services (AFS) in repatriating the mortal remains of your loved one and the expected conduct of you and your family as clients.

  • We need the following from the Family:

    1. Clothing

    2. Passport - This is so that the person can travel.

     

    AFS commits to the following responsibilities to ensure the repatriation of your loved one:

    1. Transfer to Mortuary:

    AFS - Transfer the remains of 

    2. Timely Repatriation:

    AFS - Endeavor to place the deceased on a flight within 5-8 business days, utilising our mortuary and medical experts.

    • AFS - Note that an earlier or faster time frame is not possible.
    • The process begins once the remains are collected from the Coroner or Hospital.
    • Every deceased person is different, they reach us in different conditions.
    • They are then inspected and a proper plan of preparation is commenced.

    3. Documentation Requirements:

    You the family representative - as the point of contact, must promptly complete the Death Certificate Data Collection Form to apply for the Death Certificate.

    • AFS - Flights can only be booked once the Department of Births, Deaths, and Marriages registers the death and issues the death certificate.
    • AFS - No procedures on the remains will commence without the death certificate.

    4. Medical Procedures:

    • AFS - Perform necessary medical procedures, including bathing, clothing, embalming, and sealing the body.
    • AFS - Each body is unique and will be treated individually, with varying time requirements.

    5. Documentation and Permissions:

    • AFS - Complete all necessary documentation for transporting the remains.
    • AFS - Coordinate with all relevant authorities to ensure safe passage home. 

    6. Communication:

    • AFS - Keep you informed with relevant information throughout the process.

    7. Acknowledgements:

    • You the family representative - Acknowledge that AFS has no influence over Government Departments, Airlines, or Australian Border Customs.
    • You the family representative - Understand that AFS works with third parties (e.g., freight agents) and is not responsible for delays caused by them.
    • You the family representative - Recognize that flight delays and cancellations are beyond AFS's control.
    • AFS will notify you and arrange the next available flight if this occurs.

    8. Information Provision:

    • You the family representative - are responsible for providing AFS with any required information promptly.
    • You the family representative - are responsible for filling out this form as soon as possible: Death Certificate Data Collection Form

    9. Schedule Changes

    • Understand that any schedule changes by the family may affect the repatriation timeline. Factors include but are not limited to:
      • Bringing the body home for a viewing.
      • Additional charges may apply for viewings, which will be disclosed at the time of request.
