Player/Participant Information

 

If Player/Participant is under 18 years of age, the remaining portion of this form must be completed by a parent or legal guardian.

    On behalf of myself and/or the above-named participant understand and hereby affirm relative to my participation in any activity that involves physical exercise or exertion that I am to follow the advice of my personal physician, including any restrictions, recommendations, or instructions which my personal physician has provided. I understand that the risk of physical injury is inherent with all athletic activities, and I assume such risk by participating. I am freely and voluntarily participating in physical activity which involves physical exercise and/or exertion at the facilities located in Jacksonville, North Carolina.

    I, on behalf of myself, my beneficiaries, heirs and assigns covenant and agree to indemnify, protect, defend and save harmless The City of Jacksonville, The Difference180 Recovery, The Door Christian Fellowship Church, Christian Fellowship Ministries, their agents, representatives, employees (former, current, or future employees), members, officers, directors, officials, successors and/or assigns from and against any and all damages, judgment suits, actions, proceedings, costs, disbursements, and/or expenses, including, without limitation, all reasonable attorneys' fees which may now or in the future be undertaken, suffered, paid, awarded, assessed, imposed, asserted or otherwise incurred by The City of Jacksonville, The Difference180 Recovery, The Door Christian Fellowship Church, Christian Fellowship Ministries, their agents, representatives, employees (former, current, or future employees), members, officers, directors, officials, successors and/or assigns relating to, resulting from, or arising out of my use and/or participation at the facilities located in Jacksonville, North Carolina, including but not limited to any loss or liability to my person or property, or to that of any other person or property. 

    I understand that video and/or photographs may be taken of the event in which I am (or the above-named is) participating in, at the facilities located in Jacksonville, North Carolina, and that by registering my name for this event, and providing my signature on this document, I am giving my permission (for myself, or the above- named participant) to be photographed and/or recorded on video. Furthermore, I hereby grant permission to The Difference18 Recovery the unrestricted right to use the above-named participant's picture or image in any advertising, literature, or media in which they see fit.

Tournament Rules and Regulations

1.  All Players must be at least 15 years of age. (Under 15 will only be allowed by the Director’s approval process with parents.)

2.  Teams comprised solely of minors (15-17) must have an adult (18 or older) present during all games in which that team participates.

3.  Teams will play 3 on 3 and may have 1 substitute player for each team.

4.  Substitutions will be allowed only on a dead-ball.

5.  Each team must designate a team captain.

6.  Each basket will be worth 1 point and 3-pointers will be 2 points.

7.  Ball must be taken back following a basket made by opposing team or change of possession if the ball hits rim or backboard, must be taken back to the 3-point line. 

8.  Each team will be allowed two 60-second timeouts per game (clock will be stopped).

9.  Fouls will be called and the player will have 1 foul shot if it was a shooting foul. (Clock will be stopped during the foul shot call.)

10.  Game play will be tournament style with the winning team advancing to the next round (If time allows and tournament officials see fit, tournament may be played in double-elimination format). 

11.  Each game will be played up to 15 minutes or 15 points, whichever comes first (Game times or point limits may be increased at the discretion of tournament officials; teams will be informed prior to the start of the tournament).

12.  The following rules will be enforced with penalties up to and including team disqualification, player ejection, loss of points, and/or forfeiture of game:
    a.  Use of profanity will not be tolerated.
    b.  Unsportsmanlike play or unnecessary roughness will not be tolerated.
    c.  Shirts or tank-tops must be worn at all times.
    d.  Pants or shorts must be worn at or above the waist.
    e.  Hanging on the rim is prohibited.
    f.  Violent actions including fighting will not be tolerated.

13.  The game officials designated by The Difference180 Recovery Inc. will make final determinations regarding the legality or legitimacy of all player/participant actions during games including all scoring.
