PARENTAL CONSENT, RELEASE OF LIABILITY, AND INDEMNITY AGREEMENT

In consideration of services performed by Steamforce LLC. (dba Funtastik Labs Sugar Land), a Texas Limited Liability Company, who is owner and operator of Funtastik Labs at 13741 Southwest Freeway, Sugar Land, TX 77478, herein referred to as Company, today and for up-to one year from today's date, I hereby execute this Parental Consent, Release of Liability, and Indemnity Agreement (the “Agreement”) on behalf of myself and any children, relatives, or dependents identified herein (collectively ‘Participants’), and hereby agree as follows:

I and Participants are solely responsible for the supervision of my children at the Company facility and agree that Company assumes no responsibility for their supervision. I further agree that it is my responsibility to review and comply with the safety guidelines and policies of the Company and assist with ensuring my children’s compliance.

I and Participants shall abide by all Company rules, instructions, and the directions of Company employees and representative. Failure to abide by those rules, instructions, and directions can lead to the immediate revocation of my rights to use Company services and facilities, without any right to refund or any payment.

I and Participants grant exclusive permission to Company, Funtastik Franchising, and affiliated companies, to use my or Participants’ names, faces, likenesses, voices, artworks, activities, presentations, creations, or appearances in television broadcasts, media productions, photographs or videos for educational and promotional purposes such as, but not limited to, brochures, flyers, website, and social media without compensation and without restrictions as to frequency and duration.

In the event of a medical emergency, I and Participants authorize Company to transport my children to a hospital or emergency room for medical treatment and hold Company harmless in the execution of such. I agree to be responsible for any associated transportation and medical expenses.

In consideration for being permitted access to Company facilities and activities, I and Participants voluntarily assume all risk of injury from participating in Company activities and agree to fully and forever waive, release and discharge Company, its affiliates, owners, members, investors, representatives, employees, instructors, contractors, landowners, landlords, affiliated companies or entities, successors, heirs, and assigns, of any liability or obligation that may arise out of or in connection with my activities or the activities of others while at Funtastik Labs.

By executing this agreement, I declare under penalty of perjury that I am doing so only for myself and/or Participants for whom I am authorized. If I or Participants or any legal representative files a claim for lawsuit out of my or Participants' participation in Company, I agree to defend, indemnity and hold harmless Company.

I understand the terms of this PARENTAL CONSENT, RELEASE OF LIABILITY, AND INDEMNITY AGREEMENT and understand that this release of liability will prevent me and Participants from filing suit or making any claims for damages in the event of any loss or injury arising from participation in Funtastik Labs activities.
