ART CLUB

Thank you for your interest in the Carver Art Club! My name is Hailey Glaze and I am the sponsor of CAC as well as the art teacher at Carver. As the sponsor of CAC, my mission is to offer students a safe space where creative thinkers of all abilities can explore, create, and engage in the creative process. I love sharing creative experiences with my students through art making. Here are a few things to consider before completing your application:

• There are only 30 spots available and it is first come, first served.
• The materials fee is $32 and can be paid via MyPaymentsPlus.
• The fee and application is what secures your spot on the roster so ensure those are gotten to me in a timely manner.
• The fee covers all the supplies we will use and your art club t-shirts. We will meet twice a month after school from 2:30-4:00 in the art room (Room 256)
• You are responsible for making sure you have timely transportation.
• If you miss a meeting, it is your responsibility to pick up your supplies from me to complete the project on your own time.
• Please bring a snack if you would like! No food will be provided.

Both student and guardian please sign below to state that you have read the application and understand the expectations of the Carver Art Club. The goal of Art Club at CMS is to open the art studio for students to come and explore various art making processes in a safe and imaginative environment. To ensure this can happen, the following is expected of all students:

5. I understand that it is my responsiblity to keep up with the meeting dates.

6. I understand that the meeting dates and times for Art Club can be found on the CMS website, in the announcements, as well as in my email.

7. I understand that pick up time is promptly at 4:00 in the bus lane.
