Welcome to the 2024-2025 Youth Programs Registration Form. This single registration form will allow you to register your children for all ministries, activities, and education programs available at Holy Apostles Church for the 2024-2025 Ecclesiastical Year. Please note that in order for your child/children to participate this year (regardless of past participation), you must complete this form in full. If you have any questions, please contact us HERE, and we will direct your inquiry to the appropriate ministry leader. Authorization Form

In consideration of the opportunity for my child/children to participate and fully realizing that such an undertaking involves an element of risk, we assume all risks and hazards incidental to such participation and do hereby release, indemnify, and agree to hold harmless Holy Apostles Greek Orthodox Church, its agents, volunteers and officers, and the chaperones, leaders, organizers, and sponsors. Neither Holy Apostles, Greek Orthodox Church, nor any of said persons shall be held financially responsible for any injury, illness, or death incurred during youth activities as a direct or indirect result of this activity.

Youth Behavior Agreement

Please go through the following information below with your child(ren)

As a participant in Holy Apostles' Youth Programs, your child(ren) agree(s) to the following:

1. Regardless of my age, I agree I will not have in my possession: alcohol, tobacco, nicotine-containing products, vaping, marijuana, illegal substances, and/or drugs during any Parish Youth Activity.
2. I also agree to leave at home any items which are weapons.
3. I agree to respect the authority of the staff, following their directions and accepting their decisions.
4. I agree to respect the needs and feelings of others, to show kindness to all I come in contact with, and to help out whenever I can.
5. I agree to respect all property including in the sanctuary, classrooms, and all parts of the building indoors and outdoors at Holy Apostles or any location where our youth are visiting, as well as any personal property belonging to others.
6. I will avoid being distracted by my phone or electronic devices during youth activities by keeping these items put away unless given express permission to use them.
7. I agree to pursue the fun and enjoyment of these youth activities wholeheartedly and in the spirit of Christian fellowship!
8. Any violation of these agreements will be dealt with at the discernment of the ministry advisor in consultation with the parish priest.

Free-Will Donation

Holy Apostles relies on the generosity of our parishioners—therefore, we invite you to make a free-will donation to support these fun and spiritually edifying ministries our parish provides to your family. Please note there will no longer be a registration fee for Sunday School. We consider educating children to be an important ministry of our parish but encourage families to consider making a free-will donation or increasing stewardship contributions.
