  • I hereby grant permission to Ken's Beauty Bar to take photographs and/or videos of me during my appointment. I understand that these images may be used for promotional purposes on Ken's Beauty Bar's social media pages, including Instagram.

  • I acknowledge the potential risks of receiving makeup services at Ken’s Beauty Bar. I promise not to hold the company, its officers, agents, or employees liable or pursue legal action against them in the event of an unexpected reaction to the makeup products used. This waiver absolves the company of any duty for reactions or injuries sustained before, during, or after the makeup service.

    By signing this agreement, I agree to hold Ken’s Beauty Bar completely harmless, including financial responsibility for any reactions or injuries sustained, regardless of the cause or circumstances.

    I will do all in my power to follow the instructions and safety requirements provided by the makeup artist, and I will seek clarification if necessary.
