This application must be submitted for any type of sanctioned meets.

USE THE NEW and CURRENT MEET ANNOUNCMENT posted on the website.

There will be a limit of one sanction awarded per weekend for one host club at one pool. For the purpose of this policy, a weekend is defined as Thursday through Sunday. Only one participation fee per sanction is allowed.

The sanction fee, a copy of the event information and event entry form are attached, or previously sent, and includes the warm-up procedures, which must be adhered to by all participants. As a condition of obtaining such a sanction, I and the above organization, which I represent, agree to abide and govern this event under the rules and regulations of United States Swimming, Inc. and New England Swimming, Inc. and all other term and conditions upon which this sanction may be granted. These terms specifically include all local rules and regulations and those set forth in Article 202 of the current edition of USA Swimming Rules and Regulations, specific reference to Paragraph Six of Article 202.2 thereof, which provides that:

“In granting this sanction it is understood and agreed that USA Swimming shall be free from any liabilities or claims for damages arising by reason of injuries to anyone during the conduct of the event.”

REVIEW AND ATTEST TO THE FOLLOWING

In applying for this sanctioned event on the dates listed above and at the location listed above, our club agrees to comply and to enforce all health and safety mandates and guidelines of USA Swimming, New England Swimming, and the State where the meet is located.

All participating athletes and clubs are currently registered with USA Swimming and/or New England Swimming. If any participants are competing from an adjacent LSC, I, as the meet host, have formally received permission from the LSC that registered the other athletes and/or clubs and have attached documentation of that permission request.

All New England Swimming members competing must be in compliance with the New England Swimming Coronavirus Waiver Policy. New England Swimming retains the right, for any changes in health and safety considerations, or in compliance with governmental or health department guidance or rules, to revoke this sanction at any time on the authority of the General Chair or the Vice Chair for Program Operations.

The undersigned certifies that the proposed facility meets all the requirements of Article 103 of USA Swimming Rules and Regulations for the requested season. In granting this sanction it is understood and agreed that USA Swimming, Inc. and New England Swimming, Inc. shall be free from any liabilities or claims for damages arising by reason of injuries to anyone during the conduct of the event.

Officials:. Each meet hosting club is responsible for securing a meet Referee, Starter, Admin Official, Safety Monitor and 2 Stroke & Turn Officials.

Officials for this meet should be qualified persons as certified by United States Swimming, Inc., and a list of such officials will be submitted one week prior to the above event, if requested.
