GENERAL AGREEMENT
IN CONSIDERATION OF the acceptance of this entry and the opportunity to participate
and/or to be judged in the Cynosport Flying Disc Championship held on the dates to which
this entry form applies (the "Event"), I UNDERTAND AND WILLINGLY ACCEPT THAT
THERE ARE INHERENT RISKS IN PARTICIPATING IN THE EVENT, INCLUDING BUT
NOT LIMITED TO RISKS ASSOCIATED WITH EVENTS INVOLVING DOGS.
I AGREE TO HOLD HARMLESS each of the parties listed below (including each of their
owners, members, officers, directors, agents, volunteers, and employees), hereinafter
collectively referred to as the "Event Organizers", from any claim for loss or injury that may
have been caused directly or indirectly to any person or thing by the act of myself or the dog
listed on the entry form while in or upon the event premises or grounds or near any entrance
thereto -
Organizing Committees of 2024 Cynosport® World Games and the Cynosport Flying
Disc Championship
Anthem K9 Sports as organizer of Cynosport Flying Disc Championship
United States Dog Agility Association, Inc. ("USDAA"), World Cynosport Limited and
affiliated companies, as organizers and producers of 2024 Cynosport World Games
Property Owners of the premises upon which the event is held, namely Pima County and
Its Districts
Sponsors of the aforementioned parties
I WAIVE RIGHT OF CLAIM, COVENANT NOT TO SUE, AND AGREE TO HOLD
HARMLESS the aforementioned parties from any claim for loss arising from property
damage or as the result of death, injury or illness to myself and the dog listed on my event entry form, or other dogs under my care that are present at the Event, without regard to the cause of any such loss.
I FURTHER WAIVE RIGHT OF CLAIM AND AGREE TO HOLD HARMLESS THE
AFOREMENTIONED PARTIES for any claim for loss that may be or have been sustained or
incurred as a result of cancellation of this event and any series of events of which this event
is a part by reason of war, terrorism, earthquakes, hurricanes, acts of government, plague, pandemic, war, or other circumstances or events beyond the control of the parties.
THROUGH ENTRY TO THIS EVENT I UNDERSTAND, ACKNOWLEDGE, AND ACCEPT
ALL RESPONSIBILITY AND LIABILITY FOR ANY LOSS SHALL BE MINE AND MINE
ALONE.
Media Release, Grants & Waivers of Rights to Broadcast & Publish
COMPETITOR HEREBY GRANTS TO USDAA and its affiliated companies, representatives, agents and assigns, all rights and permission to use or appropriate his/her and their dogs name, biography, likeness, photograph, voice, performing persona, or other indicia of identity for broadcast, telecast, cablecast, transmission or distribution in any format or media known now or in the future.
Waiver of Rights to Privacy, Publicity, & Personality
FURTHER, COMPETITOR HEREBY RELEASES United States Dog Agility Association, Inc. and its affiliates, representatives, agents and assigns from any claim or cause of action for invasion of the rights of privacy, right of publicity, right of personality, or any similar right.
Signature/Electronic Submission
THROUGH SUBMISSION OF AN ELECTRONIC ENTRY FORM TO THE EVENT, I
ACKNOWLEDGE, ACCEPT AND AGREE TO ABIDE by the provisions of this General
Agreement, and all policies, rules, regulations and conditions of entry to the Event.
