Understanding of Risks and Procedure:
IN CONSIDERATION of the risk of injury that can occur while participating in MV Beauty Artistry permanent jewelry services.
IN CONSIDERATION of my desire to participate in said activity and being given the right to participate in same; I HERRBY, for myself, my heirs, executors, administrators, assigns, or personal representatives (hereinafter collectively, "Releasor", "I" or "me", which terms have also include releaser's parents or guardian if Releasor is under 18 years of age), knowingly and voluntarily enter into this WAIVER AND RELEASE OF LIABILITY and hereby waive any and all rights, claims or causes of action of any kind arising out of my participation in this activity.
I am voluntarily participating in the aforementioned activity and I am participating in the activity entirely at my own risk. I am aware of the risks associated with participating in this activity, which may include, but are not limited to, physical or physiological injury, pain, vision impairment, emotional loss, and the possibility of the jewelry becoming loose or falling off. I acknowledge that while MV Beauty Artistry uses quality products, ones skin oils or body PH can cause the jewelry to discolor. I also acknowledge that although it is called permanent, I need to handle my jewelry with care as it is still a delicate piece of jewelry that can stretch out or break if pulled/caught on something. I assume all related risks, both known and unknown to me, of my participation in this activity.
I FURTHER AGREE to indemnify, defend and hold harmless MV Beauty Artistry against any and all claims, suits or actions of any kind whatsoever for liability, damages, compensation or otherwise brought by me or anyone on my be half, including attorney's fees and any related costs.
I confirm that I have made aware of any allergies I may have that may affect my permanent jewelry wear. I understand not to look directly at the spark during the welding process. Doing so, could severely affect my eyesight. I understand that any sudden movement could cause the welder to scratch or burn my skin. I acknowledge that I must sit still and not look directly at my permanent jewelry as it is being welded. I confirm that I am age 18 or older and am able to legally consent to participate.
Consent and Release:
I hereby acknowledge that I have carefully read this waiver and release and fully understand that it is a release of viability. I expressly agree to release and discharge MV Beauty Artistry and all its affiliates from any and all claims or causes of action. And I agree to voluntarily give up or waive any right that I otherwise have to bring a legal action against MV Beauty Artistry for personal injury.
WARRANTY & CARE POLICY:
MV Beauty Artistry is NOT responsible for lost or broken chains/charms/connectors. MV Beauty Artistry will reweld a jump ring at no cost if your jewelry from us breaks at the welded point on the jump ring. Humidity, salt water, chlorine, perspiration, skin products, household chemicals, and body pH levels can tarnish your jewelry. Usually, jewelry can be cleaned and polished to restore original appearance. However, MV Beauty Artistry is not responsible for tarnished jewelry. MV Beauty Artistry is not responsible for allergic reactions or irritations under any circumstance. MV Beauty Artistry upholds a NO REFUND policy.

Q: What is Permanent Jewelry? A: Personalized chains are welded onto the wearer. This replaces the need for a clasp. Q: Does it hurt? A: No. We use a leather patch to create a barrier between your skin and the low temp spark. Q: Can I take it off? A: Simply snip the welded ring with scissors to remove your jewelry. Save it, and we will reweld it when you're ready! Q: Does it require maintenance? A: Cleanse the jewelry with a mild dish soap, warm water, and a soft brush if you notice any build up or debri. Q: How long does it last? A: You can expect several years with proper care depending on the type of metal you choose. Q: Will you replace my permanent jewelry if it is lost or broken? A: No. Lost or broken chains will not be replace. If you buy a bracelet from Macy's and you loose it, you would not expect them to replace it. However, if your jewelry breaks on the weld point and you have the jewelry to prove it, we will gladly reweld for free.
Photo Release:
I, {fullName_1} hereby grant MV Beauty Artistry the right to use and reproduce any photographs, video footage, or audio recordings in which I appear or am heard, taken by MV Beauty Artistry or their representatives, for the purpose of promotion, advertising, educational materials, etc.
I understand that these photographs, video footage, or audio recordings may be used in any medium, including marketing, print, electronic, or online media, and may be edited or modified at the discretion of MV Beauty Artistry.
I hereby release MV Beauty Artistry, their employees, officers, directors, representatives, and agents from any and all claims, demands, and causes of action that I may have against them in connection with the use of these photographs, video footage, or audio recordings, including any claims for invasion of privacy, defamation, or right of publicity.
I understand that I will receive no compensation for the use of these photographs, video footage, or audio recordings.
I am at least 18 years of age, and I have read and understand the terms of this release. If I am under the age of 18, my parent or legal guardian has read and understands the terms of this release and has given their consent for me to sign it.
