You will receive an email copy of this form when you submit it; we recommend you print that copy for your records.   • Camper Code of Conduct

    Girl Scouts of Connecticut summer camp has rules to follow for everyone to ensure all campers have a fun filled safe experience. The Camper Code of Conduct, combined with the Girl Scout Promise and Law is a guide for expected behaviors at Girl Scout camp and at bus stops and while in transport.
  • CAMPER: I agree to abide by the Camper Code of Conduct while at camp and when riding the bus to and from camp.

    I will do my best to be honest and fair, friendly and helpful

    I will tell the truth and listen, speak friendly to all and help when needed

    Considerate and caring, courageous and strong

    Be kind, polite and wait my turn, try something new, participate in camp activities and challenge myself

    Responsible for what I say and do, respect myself and others, respect authority

    Think before I speak and act, be a positive role model, listen to the camp staff and ask for help when needed

    Use resources wisely, make the world a better place

    Respect the camp property by cleaning up after myself, participating in kapers (camp chores), ask permission before taking or using something and look for ways to make each day better

    Be a sister to every Girl Scout

    Treat everyone as I would like to be treated and be a friend to others

     

    I understand the following are unacceptable behaviors at camp:

    · cursing, name calling, or verbally abusing anyone for any reason

    · display of jewelry, clothing, accessory or manner of grooming related to gangs, or illegal substances

    · fighting, threatening, stealing, damaging property, or endangering the well-being of myself and others

    · discussing, viewing, or engaging in sexually explicit matters

    · not keeping my hands to myself and not touch, hit or strike anyone for any reason

    I understand that all prescription and over the counter medications, inhalers, injectables, creams, lotions, drops and vitamins must be turned in to the Health Care Director upon arrival at camp.

    I have read and understand the Camper Code of Conduct and agree to uphold this agreement. I understand that if I am unable to follow these rules that my Parent/Guardian will be contacted and I could be sent home from camp by the camp director.

     I understand the following are NOT allowed at any Girl Scout summer camps: pets/animals, tobacco products, weapons, alcohol, illegal substances, personal sports equipment (unless specified for a camp program), any electronic devices such as but not limited to: i-pods, tablets, cell phones, etc. I understand that electronic devices cannot be used at camp. (Girl Scouts of Connecticut is not responsible for loss or damage to electronic devices or personal sports equipment).

    I have discussed the above with my camper and feel that they understand what is expected while participating at Girl Scouts of Connecticut summer camp.

    I also understand that should they be unwilling or unable to follow these guidelines, I may be called to pick up my child at camp.  Should I be asked to pick up my child from camp, I am responsible for picking up within four hours (from the time of the request) at day camp and within 24 hours (from the time of the request) at resident camp. If a parent/guardian fails to collect his/her child within the specified time frame, the child will be reported as abandoned to the local police.

    I further acknowledge that should the participant be asked to leave due to her behavior there is no refund.

    Please provide accurate medical information to ensure appropriate care and treatment for your camper.
  • Camper Medical History

    This information is reviewed upon receipt and reviewed again by the on-site summer camp health team within 2 weeks of your camper's arrival at summer camp.
  • Allergies

    Camper allergy information is shared with staff at summer camp as needed for the health and safety of your camper. Be sure to list known allergies, reactions and treatment plan(s) as needed.

  • If your camper has special services in or out of school of which summer camp should be aware or that might impact the camp experience (I.e. personal care assistant, health aide, regular counseling, etc.), or an Individual Plan of Care, please also complete the IEP Camp Plan form. 

     

  • Camper Physical Documentation

    All campers are required to submit a copy of their latest health physical (the form generally provided to schools or, the "blue form"). The date of exam on the physical submitted must have been within 36 months (3 years) from the FIRST day the camper is at camp.

  • Girl Scout Permission and Activity Waiver

    Girl Scouts of Connecticut camps and outdoor program centers frequently have rustic and uneven terrain that may include hills, woodlands, and waterfront areas whose navigation may require concentrated physical exertion (such as walking, hiking, or swimming). Many of the programs/activities described in camp or program literature and provided by Girl Scouts of Connecticut include a variety of physical activities in the outdoors. By signing this consent for your Girl Scout to participate, you acknowledge that their participation in the program activities, as described in the camp brochure and other materials, requires an acceptance of possible serious injury to your camper. You have made your own judgement that your camperis capable of participating in activities at camp. You also are representing to Girl Scouts of Connecticut that your camper has no undisclosed condition that might affect their fitness or ability to participate with or without reasonable accommodation. 

         In any program/activity at summer camp, there is always some risk of personal injury or property damage. In return for the agreement to permit your camper to participate, you agree that you will not sue Girl Scouts of Connecticut, (hereafter the “council”) and its members, employees, agents, assigns or volunteers for any injury to your camper that results from or may aggravate the inherent risks of your camper’s participation in a program/activity at summer camp. The inherent risks involved in participation in any program/activity include those injuries which result from circumstances or actions of persons who are not employees or agents of the council. 

         Please read this agreement carefully and make certain that you understand it fully before signing it.  By signing below, you may be giving up substantial rights that belong to you and your Girl Scout camper.  You agree that you are signing this consent freely and voluntarily. My camper is in good health and can fully participate in all general activities.

  • Signature and Authorization for Medical Treatment

    I hereby authorize the Girl Scouts of Connecticut (GSOFCT) and any medical personnel selected by the camp to provide medical assessment and routine medical treatment and services to my camper, including hospitalization, and necessary related transportation, and in case of an emergency, authorize the provision of medically necessary treatment/services, including transfer to a hospital or facility for emergency treatment/services.  I release GSOFCT and its officers, directors, employees, personnel from and against any and all claims and liability arising from or related to the provision, authorization and administration of medical treatment, services and medication to my camper.
