• Burnside Paws Animal Rescue Dog Adoption Application

  • Burnside Paws Rescue is an all-volunteer, non-profit, animal welfare organization dedicated to the rescue and placement of stray, neglected and abused animals mostly dogs listed for euthanasia. Burnside Paws Rescue is completely funded through fundraising efforts and public donations. 

  • This questionnaire is designed to help Burnside Paws Rescue and yourself determine if a specific dog is the right one for you and your household. All adopting parties are required to complete this form prior to adoption. Burnside Paws Rescue reserves the right to deny any adoption based on answers to this questionnaire and/or interviews prior to adoptions!

     

    Please Note that $600 upfront will be required to any dogs being requested to pull off of the euthanasia list. If you agree to this, we look forward to helping you adopt your new best friend.

    By submitting an application to Burnside Paws Animal Rescue person(s) recognize application will be reviewed and processed by members of Burnside Paws Rescue. Applicant grants permission for listed veterinarian to release medical information to Burnside Paws. Burnside Paws may utilize information submitted to do a compliant DMV and criminal background check. Any references are granted permission to speak to a representative of Burnside Paws doing background for application to adopt a rescue dog *
