  • Parental Consent Form

  • I understand that participation in the activity involves a certain degree of risk. I have carefully considered the risk involved and have given consent for myself or my child to participate in the activity. I understand that participation in the activity is entirely voluntary and requires participants to abide by applicable rules and standards of conduct. I release Alpha Phi Alpha Fraternity, Inc., the chapter, the activity coordinators, and all employees, volunteers, related parties, or other organizations associated with the activity from any and all claims or liability arising out of this participation. In case of an emergency involving my child, I understand every effort will be made to contact me. In the event I cannot be reached, I hereby give my permission to the medical provider selected by the adult leader in charge to secure proper treatment, including hospitalization, anesthesia, surgery, or injections of medication for my child. Medical providers are authorized to disclose to the adult in charge examination findings, test results, and treatment provided for purposes of medical evaluation of the participant, follow-up and communication with the participant’s parents or guardian, and/or determination of the participant’s ability to continue in the program activities.

  • Code of Conduct

  • 1.      Respect all participants (other youth and adult volunteers) by not using foul, hurtful or obscene language or engaging in physical violence, bullying (including cyber-bullying) or other aggressive behaviors that threaten the safety of others.

    2.      Respect the property rights of others.

    a.      Do not damage or deface the building or property within the building where chapter activities are held.

    b.      Do not damage or take the personal property of any other participant or volunteer; and do not use Alpha Phi Alpha’s name or any symbol or logo (Alpha Phi Alpha’s branding) associated with the fraternity on any clothing, books, bags, or other items.

    3.      Return supplies to their proper place after using them.

    4.      Clean up all work areas properly.

    5.      Listen carefully to directions and when someone else is talking.

    6.      Respect designated quiet areas, such as homework/reading area.

    7.      Stay within the program’s designated areas within the building.

    8.      Cooperate and participate in organized activities.

    9.      Assume full responsibility for all personal belongings. Please leave valuables at home.

    10.  Do not bring any weapons, cigarettes/drugs, alcohol, or anything illegal to any activity at any time.

    11.  Do not use electronic devices without the permission of the program leader.

    12.  Refrain from making disparaging comments about others regardless of if they are present or not.

     

    Sanctions for Violating the Code of Conduct

     

    Poor Conduct, including degrading language, Inappropriate use of electronics, verbal Harassment, etc.:

    1st Occurrence: Verbal warning, parent or guardian notified verbally or electronically.

    2nd Occurrence: Loss of access to the program for two weeks. Written letter to parent/guardian.

    3rd Occurrence: Loss of access to the program for one month. Final letter to parent/guardian.

    Next occurrence youth is removed from the program.

    Physical Violence, Bullying, and Other Misconduct:

    1st Occurrence: Removal from the situation, loss of privileges, guardian notified verbally or electronically.

    2nd Occurrence: Youth is removed from the program

    Illegal Substances or Dangerous Weapons, Sexual Misconduct

    1st Occurrence: Youth is removed from the program. Police will be notified.

  • Member / Volunteer Code of Ethics

  • All members and any volunteers working with participants in Alpha Phi Alpha Fraternity, Inc. Youth Engagement

    Programs are expected to embody the highest standards of integrity. This Code of Ethics embodies the affirmation of your commitment to follow tenets that are integral to the fraternity’s Youth Engagement Programs:

    1.      I will treat youth with respect, care and acceptance. I know that all young people are valuable and capable of helping others and improving their communities. I will use a democratic approach when working with youth.

    2.      I will honor my volunteer commitment. I will strive to live up to my volunteer commitment by working the hours necessary to fulfill the volunteer role I have accepted.

    3.      I will seek training for my volunteer role. I will participate in meetings, self-study or other training opportunities, which will help me work more effectively with youth and adults.

    4.      I will provide a safe environment. I will not harm youth or adults in any way, whether through sexual harassment, physical force, verbal or mental abuse, neglect or other harmful activities.

    5.      I will abstain from using alcohol or any illegal substance while working with, or while responsible for youth; neither will I allow youth to use any such substance while under my supervision. For states where substances, such as marijuana are legal, I will abstain from use while working with or while responsible for youth.

    6.      I will obey the laws of the locality, state and nation.

    7.      I will strive to be a positive role model. By my example, I will help youth learn to respect and cooperate with others. I will teach others to compete honestly and fairly.

    8.      I will work as a “team player” for the good of all persons. I will work cooperatively with other adult volunteers for the good of all involved in the Alpha’s Mentoring.

    9.      I will work within the Alpha Phi Alpha Fraternity system. As a volunteer, I am accountable for my actions. If my personal conduct is deemed to be in violation of any of Alpha’s policies, I understand I may be relieved of my volunteer role.

    10.  I will not have outside contact with any youth participant; without the expressed written permission from the parent/guardian or the parent/guardian is physically present during the outside contact.

  • PHOTOGRAPH & DIGITAL MEDIA AUTHORIZATION RELEASE FORM

  • I (“Parent/Guardian”), as a parent or legal guardian of {menteesName01}, give permission for Eta Eta Lambda Chapter of Alpha Phi Alpha Fraternity, Incorporated (the “Chapter”) to publish on the Internet or media still photographs or moving images, including, if applicable any sound recordings accompanying the images (“Images”) taken of my child during participation in Youth Engagement Program activities, without payment or any consideration and without notifying me in advance. I also give permission for the Chapter to highlight my child’s achievements and activities in efforts to promote the Youth Engagement Program through newspapers, radio, TV, electronically, displays, brochures, and other types of media without payment or any consideration and without notifying me.

     

    I understand and agree that these Images will become the property of the Chapter, which shall have complete ownership of the Images. I hereby irrevocably authorize the Chapter to publish or distribute these Images for the purpose of publicizing the Chapter’s programs or for any other lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my child’s likeness appears. Additionally, I waive any rights to royalties or other compensation arising out of or related to the use of the Images.

     

    I hereby hold harmless and release and forever discharge the Chapter and any of its officers and members; Alpha Phi Alpha Fraternity, Incorporated; its officers; National Executive Board; employees; members; representatives; agents; and assigns from any and all claims, costs, suits, actions, judgments, and expenses which my child, his, representatives, executors, administrators, or any other persons acting on his behalf have or may have by reason of the use of the Images. This release specifically includes, without limitation, a complete release and discharge of any liability by virtue of any editing, distortion, alteration, or optical illusion, whether intentional or otherwise, that may occur or be produced in the taking of or editing of said Images, unless it can be shown that such was maliciously caused, produced and published solely for the purpose of subjecting my child to conspicuous ridicule, scandal, reproach, scorn and indignity.

     

    I hereby certify that I am the parent/guardian of the mentee referred to above and is authorized legally to give this consent, and do hereby give my consent without reservation to the foregoing on behalf of my child.

  • Youth Athletic Event Participant Waiver

  • I, {menteesName01}, As a registered participant in an activity sponsored by Eta Eta Lambda Chapter of Alpha Phi Alpha Fraternity, Inc., I understand and agree that I am participating in this event on my own free will and accord and that neither Chapter, nor Alpha Phi Alpha Fraternity, Inc., nor its insurer(s) will share in or accept responsibility for any liability for bodily injury, property damage, medical expense or other loss that may arise from my participation in this event.

     

    I further understand and agree and have no expectation that Eta Eta Lambda Chapter, or Alpha Phi Alpha Fraternity, Inc. will provide any form of security or another measure of safeguarding for this event, as there is no reasonable expectation that such will be necessary.

     

    I further understand and agree that this event is considered a “no-fault” event by me, as well as Eta Eta Lambda Chapter, and Alpha Phi Alpha Fraternity, Inc., and in the event of bodily injury, property damage, the necessity of medical expenses or other loss, I agree to incur my own expenses without input or participation from Eta Eta Lambda Chapter, or Alpha Phi Alpha Fraternity, Inc., or its insurer(s).

  • Field Trip Waiver

  • To be filled out by parent:

     

    I grant permission for {menteesName01} to attend the Alpha Academy field trips. I expect my child to be responsible for his own actions during this event and travel to and from it, to be a cooperative member of the group so that this activity can be a wholesome means of fellowship.

     

    I have read the statement of responsibility above and have talked or will talk with my child about it. The Alpha Academy and adult leaders are held with no liability for unwise actions on my child’s part.

  • In case of emergency, I understand that every effort will be made to contact me. If I cannot be reached, I hereby give the designated sponsor permission to act in my behalf in seeking emergency treatment for {menteesName01} in the event that such treatment is deemed necessary by the designated sponsor.

     I give permission to those administering emergency treatment to do so, using those measures deemed necessary.
