By filling out this form, you are requesting to try the next available date for the selected class. You will receive a confirmation email with the date of the upcoming trial class within 1 business day. We ask that you request your trial class a minimum of 1 business day in advance. If the class that you would like to try is not listed bellow, please email us at dance@northernplainsdance.org.

Once you have submitted this form, you will receive an email confirmation detailing which upcoming class you have selected as your trial class.

This form does not enroll the above student in Northern Plains Dance classes. If the student wishes to continue dance training with Northern Plains Dance, a full registration form must be completed and submitted with payment.

By submitting this form, you are agreeing to the following policies:

I hereby release Northern Plains Dance or Ballet (NPD), its agents and employees from all liability for personal injury, illness, or property damage occurring on or off premises used, rented, leased, or owned by NPD. I certify that I am in good health and capable of participation in all activities and classes. In an emergency, I authorize NPD, its employees and agents, in the event that I cannot be reached by telephone at the number listed above, to transport and admit my child or myself to a local hospital for the purpose of emergency medical treatment. I release NPD, its employees and agents, from any liability incurred for the transportation and admission of myself or my child to a local hospital for emergency treatment. I hereby give permission to NPD to take photographs and/or videos of me or my child that will permanently become property of NPD. I consent to the use of such materials for promotional purposes by NPD.

WAIVER OF LAWSUIT/LIABILITY: I hereby agree to save, hold harmless and indemnify NPD, its officers, directors, managers, officials, trustees, agents, employees or other representatives in connection with exposure, infection, and/or spread of COVID-19 or any other illness related to participating in NPD activities. I understand that this waiver means I give up my right to bring any claims including for personal injuries, death, disease or property losses, or any other loss, including but not limited to claims of negligence and give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen.
