  • 3. The Pre-Job Safety Instruction Program (PSI) and the following steps been explained and reviewed?*

  • 4. Has a copy of the certification for the equipment been obtained where applicable? (Crane, Picker Truck etc), Copies of operators tickers supplied also?*
  • 5. The proper selection, care and use of the following PPE?*
  • 6. HSE signs and compliance?*
  • 7. Housekeeping requirements? (Garbage bin locations etc)*
  • 8. Have the following site-specific job hazards been reviewed? They are subject to but not limited to:*
  • 9. Ladder requirements? (secured, 3-point connection)*
  • 10. Guardrail requirements?*
  • 11. Project fall protection plan requirements?*
  • 12. Intoxicating beverages and drugs prohibited on the worksite?*
  • 13. The emergency response/evacuation procedures?*
  • 14. The incident reporting procedures?*

  • *Worker acknowledges that this orientation is specific to the activity listed in part 16 only. A full orientation will be required for anything outside the agreed scope of service or delivery work.

    This form will be retained on file at the project worksite location.
