I GIVE AUTHORISATION to transfer electronic funds on a recurring basis from the checking, savings, or credit card account I specified above for my membership dues. This authorisation is to remain effective until PHIITKID PTY LTD has collected for all charges assessed in connection with the terms and conditions of my membership agreement.
Membership Term: I acknowledge and agree that I will maintain membership and pay the dues specified. I agree that I will be liable for dues for the entire DURATION, Term of this agreement in the amounts as stated herein regardless of usage or any change of circumstance.
Next Payment: I understand my next payment is due and on the same day of each calendar month/frequency/date, I HAVE SELECTED.
Membership Renewal: After the initial (memberships) Term has expired, this Agreement shall automatically continue from month to month at the prevailing membership rate, which is subject to change, until I contact PHIITKID to cancel. I am responsible for all charges billed or incurred prior to cancellation. I further agree that my membership benefits are exclusive to me and are in no way transferable to anyone else.
FAILURE to attend the Classes does not RELIEVE me of the obligation to PAY the full tuition for the program or phase in which I am currently enrolled.
PHIITKID or any party designated by PHIITKID may photograph or film me while I attend the premises of Phiitkid and use photos, video footage and/or video streaming for promotion, sales, publicity and advertising purposes for all media platforms.
The required physical exertion may be strenuous and cause physical injury, and I am fully aware of the risks and hazards involved. I hereby represent that I am physically fit to receive and participate in the prescribed course of instruction.
I acknowledge, IF I DO SUFFER FROM ANY ILLNESS. I have made it known to PHIITKID regarding my illness. I have been given permission by a medical advisor and I am aware of risk, and limitations and-have been advised to consult with a physician prior to and regarding my participation in the Classes.
I am aware that the type of training and environment may cause harm to myself or others if I do not comply or listen to my instructors. I will comply with all the rules and regulations of PHIITKID. And if I choose to bring my child/children, I am fully responsible for their safety, and PHIITKID will not be liable for any damages/injury due to negligence.
I hereby agree that if PHIITKID is unable to process my payment from the designated account provided by me PHIITKID is permitted to use any of the other accounts information provided as many times and as often as needed to replace the unpaid payments.
I understand this agreement may be cancelled at any time by paying a cancellation fee $400 OR of the remaining term of my agreement. All discounts given will not be considered in the calculation of the cancellation fee. If the student and/or co-signer wish to cancel this agreement on the stated terms set out, they may do so by giving 2 weeks’ notice, ensuring payments are up to date, otherwise payments will continue until all payments are up to date to process cancellation.
FOUNDATION MEMBERSHIPS/KIDS MEMBERSHIPS: Must give two weeks’ notice, and ensure all payments are up to date before finalising, if you choose to return, foundation will no longer be available to you. Any outstanding amounts will be sent to a third-party debt collector to ensure all monies have been received.
6 WEEK CHALLENGE: If I do not complete the 6 WEEK challenge. I understand I am liable for any outstanding fees, Phiitkid will have the right to forward any outstanding amounts to a third-party debt collector to ensure all monies have been recovered. Prize money will be used to cover any outstanding debts.
Use of Recovery Equipment/Equipment: Sauna and Ice bath is available for use depending on the package membership you have selected. You can purchase single use also. Using the recovery area, I have made it known to PHIITKID any concerns that may affect my health or have advised of pre-existing conditions. I am fully Liable for any damages that were caused during the time of use and will notify Staff. PHIITKID will discuss payment recovery to fix/replace any damages.
Membership Holds: Memberships can be placed on hold for a maximum 2 weeks, due to family emergency or Injury (depending the situation) Doctor's certificate must be emailed to Phiitkidfitness@gmail.com I understand my Memberships cannot be put on hold if my account is overdue.

I understand my rights and obligations as stated above, and I have read and understood this agreement and I am aware that by accepting this Agreement I have accepted the terms and conditions, rules and regulations and will comply.
