This form indicates that on acceptance into the Declans Basketball club (DBC), the player (Parents of the player IF u18) participating authorizes DBC to act for them (the player) according to their (DBC) best judgment in any emergency requiring medical attention and the player hereby releases, exonerates, and discharges DBC and its employees, agents partners or subsidiaries from any and all action or causes or actions known or unknown resulting in any player injuries while at any of our events. By signing this form,you confirm the player's registration in the Basketball Ireland system. This approval also ensures that the player is insured by Basketball Ireland, and that any injuries sustained will be covered under their insurance policy. Parents and players signing this form also give permission to DBC employees and agents to use the player's image and likeness via photography and video footage for the promotional purposes of both the player and the academy.

ATTENTION PARENTS- this is ELITE basketball, Your son/daughter has been invited to participate and while parents opinions which are voiced in the proper manner are valued by DBC staff and taken on board where possible, a parent of any player should never try to canvas, influence or intervene with the running of the program at any level. Continuous complaining, Chinese whispers, Gossip, canvassing of general opinion and any general behaviour that undermines the efforts of the academy staff will not be tolerated in and way. Any such breach will result in sanctions up to the dismissal of the found parents child from the program. By signing this registration form you are agreeing to DBC parents charter and all rules and regulations contained within.
