The following information will be used to prepare your story. Your story will be read on Sunday morning by one of our Pastors.

I grant First Baptist Church of Wichita Falls permission to use my likeness in a photograph and video in any and all of it’s publications, including but not limited to all of First Baptist Church’s printed and digital publications. I understand and agree that any photograph or video using my likeness will become property of First Baptist Church and will not be returned. I understand that since my participation with First Baptist Church is voluntary, I will receive no financial compensation. I understand that typing my name constitutes a legal signature.
