NOTES:

Captains: Please complete these forms as soon as a match is canceled. This ensures a timely response from all involved.

Matches that must be rescheduled due to inclement weather should be scheduled within 48 hours of the original match date and time and played within three weeks.

If no point has been played, a substitute player may play for an originally scheduled player.

Scores may be entered into TennisLink after ALL lines have been played.
