   I agree to present my work during the Literary Arts Showcase (“EVENT”) at Kearny Street Workshop’s (“KSW”) APAture 2024 (“APATURE”), to take place at ARC Gallery & Studios (“VENUE”), 1246 Folsom St, San Francisco, CA on October 25, 2024 (“DATE”). 
2. 
   I understand that I am responsible for procuring any supplies, materials, or equipment AND for taking all steps necessary to ensure delivery to the VENUE and completion of said work by the above DATE. Should I require assistance with procuring or delivering these items, I will inform KSW at least three weeks before the EVENT with the understanding that no guarantees can be made for availability or funding.
3. 
   If attendance at meetings and/or rehearsals is required from me, I will make every effort to be present. I will be responsible for providing my own transportation to and from all APATURE-related meetings, rehearsals, and events.
4. 
   Furthermore, I consent to aid KSW in marketing APATURE amongst my network of contacts from now until its presentation. 
5. 
   For my involvement in APATURE, I understand that KSW will compensate artist/group in the amount of $100(Showcase Artists)/$500(Featured Artists). I agree to provide an invoice for the processing of this payment.
6. 
   By signing this form, I certify that the accepted work is my original creation and that I hold the rights to exhibit, present, and distribute this work at APATURE.
7. 
   I represent that I am at least 18 years of age, have read and understand the foregoing statement, and am competent to execute this agreement.

This permission and release form is for artists presenting work at Kearny Street Workshop’s APAture 2024, to take place at various venues in San Francisco from October 13 - November 9, 2024.

I, the undersigned, do hereby consent and agree that Kearny Street Workshop, its employees, or agents have the right to take photographs, videotape, or digital recordings of myself and the artwork I will present during APAture 2024 at various venues in San Francisco from October 13 - November 9, 2024 and to use these in any and all media, now or hereafter known. I further consent that my name and identity may be revealed therein or by descriptive text or commentary.

I do hereby release to Kearny Street Workshop, its agents, and employees all rights to exhibit this work in print and electronic form publicly or privately for promotional, educational, & archival purposes only.

I understand that there will be no financial or other remuneration for recording me or my artwork, either for initial or subsequent transmission or playback. Any usage of the recordings by KSW outside of the aforementioned usages must require your consent.

I also understand that Kearny Street Workshop is not responsible for any expense or liability incurred as a result of my participation in this recording, including medical expenses due to any sickness or injury incurred as a result. 

I represent that I am at least 18 years of age, have read and understand the foregoing statement, and am competent to execute this agreement.
