Salon Services Agreement:
I understand that Opal Beauty Rituals (the "salon") uses various products and procedures that may contain allergens or irritants. I agree to disclose any known allergies or sensitivities to the salon.
I understand that it is my responsibility to inform the salon of any changes to my allergy information before each appointment.
I acknowledge that the salon will take reasonable precautions, but I release the salon and its staff from liability in case of any allergic reactions or adverse effects resulting from any salon services.
I understand that a patch test may be recommended for certain services, and I agree to undergo a patch test if advised by the salon.
I acknowledge that Opal Beauty Rituals recommends a patch test before certain hair treatments, such as coloring services, to check for potential allergic reactions.
I understand the patch test involves applying a small amount of the product to a small area of my skin either on my inner elbow or behind my ear, that may stay there for up to 48 hours. This helps determine if I may have an allergic reaction to the product.
I acknowledge that it is my responsibility to request and undergo an a patch test at least 48 hours before my scheduled appointment. I understand that failure to do so may result in the cancellation or rescheduling of my appointment if I opt to have a patch test prior to any appointment. I accept that I will be held to the terms of the No Show and Cancellation Policy.
I understand that in signing this form, Opal Beauty Rituals will perform any and all future services without performing a patch test. I am aware that it is my right and responsibility to request a patch test at anytime in the future if I deem it necessary.
I release Opal Beauty Rituals, its employees, landlords and representatives from any liability for allergic reactions or other complications that may have occur if I decline or fail to undergo the recommended patch test.
