Waiver: In connection with my involvement in art creation and/or classes at Lil’ Art House, LLC , I hereby release Lil’ Art House LLC, its agents, representatives, successors, or assignees, including Lil’ Art House, LLC management, employees and volunteers, from all liabilities, actions, claims, damages, demands, costs, and expenses, which I now or in the future have against them, arising out of, or in any way connected with, my participation in art creation and/or classes, including in route to or from the program or it’s related events. I understand that participants under the age of 15 are to be accompanied by a guardian at all times. I understand that this waiver includes, but is not limited to, all injuries to me/my guests, and or loss of any personal property.

Photo/Art Release: I hereby grant Lil’ Art House, LLC permission to use my likeness and art in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration. I authorize Lil’ Art House, LLC to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product where in my likeness or art appears. Additionally, I waive any rights to the royalties or other compensation, arising or related to the use of the photo/Art. I understand that if I do not want my/my child’s photo taken, I will notify the instructor/facilitator and note my wishes here.
