Upon submitting this form, Angela will reach out within 72 hours about your tattoo, upon booking you will be asked to send a non refundable booking fee of $80.00 that goes towards the total cost of your appointment. Which of the following below do you prefer to send the booking fee?

Consent: By signing below, you acknowledge that you are of legal age to receive a tattoo, which is typically 18 years or older, or have obtained parental consent if under the legal age. You understand that getting a tattoo involves risks and potential complications, and you consent to undergo the tattoo procedure willingly.

Risks and Complications: You understand that while the staff at Definitive Beauty & Ink will take every precaution to ensure your safety, there are inherent risks associated with getting a tattoo, including but not limited to infection, allergic reactions, scarring, and dissatisfaction with the outcome. You acknowledge that you have been informed of these risks and still wish to proceed with the tattooing procedure.

Design Approval: You confirm that you have reviewed and approved the design of the tattoo to be applied. Any changes requested after the tattooing process has begun may incur additional charges.

Health and Medical Conditions: You certify that you are in good health and free from any medical conditions that may interfere with the tattooing process or healing, including but not limited to skin conditions, allergies, and immune disorders. You agree to disclose any relevant medical information to the tattoo artist prior to receiving the tattoo.

Aftercare: You understand the importance of following the aftercare instructions provided by the tattoo artist to ensure proper healing and minimize the risk of complications. Failure to adhere to these instructions may result in adverse effects on the appearance and healing of the tattoo.

Touch-Ups: Although rare, you acknowledge that the tattoo is a fine line tattoo and may require touch-ups to achieve the desired outcome, especially during the healing process. Touch-up appointments may incur additional charges, and you agree to schedule and attend these appointments as recommended by the tattoo artist.

Consent for Photography: You consent to the use of photographs or images of the tattoo for promotional purposes by Definitive Beauty & Ink, including but not limited to social media, website, and marketing materials.

Release of Liability: You release Definitive Beauty & Ink, its staff, and affiliates from any liability for any damages, injuries, or complications arising from or related to the tattooing process, including but not limited to infection, allergic reactions, and dissatisfaction with the outcome.

Agreement to Terms: By signing below, you acknowledge that you have read and understood the terms and conditions outlined above, and you agree to comply with them throughout the tattooing process.
