• New Chapter Application Form

  • As part of the Chapter Application, we will ask for the following documents to upload:

    • A roster of your elected officers or governing body 
    • Constitution, bylaws, or other governing documents 
    • Resolution

    Other required documents: 

    • ICC Members list
    • Other chapter members
    • Participating chapters (for regional chapters only)
