Welcome! We are glad you have decided to enroll your child in our Certified School Age Child Care Center. 感谢大家选择我们的儿童托管中心！

The following contract must be fully completed and signed before care can begin. After reading this contract and the policies thoroughly, please discuss concerns with us before you sign. We will agree upon fees, policies or practices before care will begin. You will receive a copy of the signed contract. 以下合同内容请在孩子上学之前填好。有问题请及时沟通。合同以英文版为准。

We provide care for children between the ages of 5 years through 13 years. 我们能接收5到13岁的学生。

Our operating hours are as follows: Monday to Friday, 2:00 pm to 5:30 pm. 运营时间是周一到周五的下午2点到5点半。

Enrollment Procedures 报名流程:
1. Not-refundable registration fee: $50 for new student, $35 for continued student.
2. No deposit fee.
3. One day free trial is offered before registration.
4. After registration approval, all families will be taken on a trial period of 5 day(s) to determine the right placement for your child. During this trial period either parent or provider has the right to terminate care with 1 day notice. You will be responsible for payment for days your child attended during the trial period
The following forms must be completed and returned to us by 8 / 23 / 2024 or before care will begin, if you are a NEW parent or your personal informations have been changed:

1. Blue card: OCFS form #0792. See file below.

2. Pick-up Authorization, Permission to Applied Insect Repellent or Sunscreen, Media Usage and Release Form.

3. Optional: Individual Allergy and Anaphylaxis Emergency Plan – OCFS 6029 (only for kids with known allergies). Ask Director for a paper copy.

4. Optional: Immunization Record (mandatory for homeschoolers.)

** A $15 fee will be charged for early drop-off and late pick up more than 10 mins.

You may be eligible for lower cost if your annual household income is less than $108,631 for a family of four and both of the guardians work in the afternoon. Contact us for more details! 如果您的年家庭收入低于$108,631（四口之家），且两个监护人在下午都要工作，您可能有资格获得政府的资助交学费。请联系我们了解更多详细信息！

5% siblings discount applies to the second child; another 10% siblings discount applies to the third child. 第二个孩子可享受5%的兄弟姐妹折扣；第三个孩子可享受额外10%的兄弟姐妹折扣。

Additional Fees/Late Fees:

You will NOT be charged a late fee for late pick up.
