Media Release Permission
The Fannin CLC may be publicized in various media/social media outlets and our staff and students participate in different forms of public media presentations. A parent or guardian must sign the permission form for a student to be a part of these publications. I hereby consent to the recording of my child's voice and likeness in print and on film, videotape, and/or audiotape and to the use of his or her name, likeness, and/or voice in any audio-visual materials on social media, radio or television broadcast and distribution or public showing of such materials in the theatrical (e.g., cinematic release) or non-theatrical market (e.g., schools, libraries, hospitals, universities, etc In connection with giving my consent, I hereby release and discharge producer and advertisers and sponsors, together with the officers, agents and employees of all those entities, from any claim or clause of action whatsoever I may have arising out of the use, broadcast, and/or publication by producer and any of its above-mentioned agents of my child's name, likeness, and/or voice.
