SURRENDER REQUEST FORM - INDIVIDUAL ANIMALS
Misty Eyes Animal Center is a non-profit, foster-based dog and cat rescue. We are not a shelter and can only accept additional animals if we have foster home space available.

If you are seeking to surrender a litter of kittens or puppies, please use the Litter Surrender Request Form. If you wish to surrender more than one animal (other than litters), please complete a separate request for each one.

It is crucial that all questions be answered to the best of your knowledge. Incomplete requests will not be processed. Please be aware that we attempt to respond to surrender requests as quickly as possible, but due to high volume, it may take several days to process your request. If you have not received a response within 72 hours, be sure to check your spam folder.
