If pet owner cannot be reached, an attempt will be made to contact the emergency contact listed above to determine course of action. If vet care is needed, we will transport your pet immedicately to receive care. Vet fees are the responsibility of the pet owner.

Vaccinations up to date?*
A copy or picture of updated vaccinations will need to be provided. Required vaccines: Rabies, Bordetella (Kennel Cough), and Distemper/Parvo. We do accept Titer Tests showing proper immunity against the diseases mentioned above.
Flea & Tick preventative current? *
Has your dog been to Day Care before? If yes, how was their experience? Please be as descriptive as possible. The more you share, the better we can care for your dog.*
How is your dog around other dogs? Please explain (Social, Reactive, Timid/Shy or Unsure). Please be as descriptive as possible. The more you share, the better we can care for your dog.*
Our environment is a very social one. If your dog is reactive or unsure, our drop-in visits in your home will be a better option for care.
Does your dog try to escape in any manner? Does your dog try to jump/climb over, dig under, or any other method, in a fenced in area?*
If your dog is an escape artist, it is highly recommended to have a GPS tracker attached to their collar. We make every effort to provide a safe environment for your dog without any methods of escape present.
Is it ok to share photos or videos of your pets on our business page and website?*
For your security, we will never post them while you are away. They will be posted at a later date after your return.
Are they on any medications, vitamins, ointments, or supplements that will need administered during the day? If yes, please explain when they need to be taken or applied, any special methods used to get your dog to take them (if necessary), or any other information that will make this process go as smooth as possible so we can make sure your pet gets what they need each day.*
Will feeding need to take place? If so, please provide how much to feed and how often.*
You only need to provide their food. We have plenty of bowls on hand.
Is there anything else we need to know about your dog? Remember, share as much as possible, the good and the bad. The more you share, the better we can care for your pet. The worst thing for us is to find out about a known behavior after the fact.
