Volunteer Registration Form

I, full name, hereby agree to accept a position as a volunteer worker for Rescue Alliance Services (herein after referred to as RESCUE ALLIANCE). Such actions or services are rendered to RESCUE ALLIANCE with generous and charitable motives. No liability whatsoever will be incurred by RESCUE ALLIANCE to anyone who performs voluntary services.

TERMS AND CONDITIONS
My Services to RESCUE ALLIANCE are provided strictly in a voluntary capacity as a volunteer and without any express or implied promise of salary, compensation or any other payment of any kind whatsoever.
My services are furnished without any employment-type benefits, including employment insurance programs, worker's compensation accrual in any form, vacations and sick time.
I will familiarize myself and comply with RESCUE ALLIANCE policies and procedures, and any partner organization policies and procedures applicable to volunteers. In particular, I fully understand that RESCUE ALLIANCE expects high standards of moral and ethical treatment of the animals under its care. I will adhere strictly to these standards in my capacity as a volunteer.
I understand that RESCUE ALLIANCE, without notice or hearing, may terminate my services as a volunteer at any time, with or without reason.

RELEASE AND WAIVER OF RESPONSIBILITY
I, full name , the undersigned person hereby declare and state that I am of legal age. I hereby declare and state that I desire a volunteer assignment with RESCUE ALLIANCE, and this I do so entirely upon my own initiative, responsibility, and risk. This may include assissting in the adoption of pets through RESCUE ALLIANCE at local PetSmart stores and I understand the neither RESCUE ALLIANCE or PetSmart is responsible for any illness or injury caused by any animals that I come in contact with during my volunteer work I agree to hold harmless and release from liability RESCUE ALLIANCE and PetSmart should I become sick or injured from any animals as a result of my volunteer work.
In consideration for being permitted to volunteer my services with RESCUE ALLIANCE, I hereby declare and state that I know serving as a volunteer for RESCUE ALLIANCE carries risk(s) of personal injury, death, and/or property damage (including damage to my personal pets) and I assume full responsibility for any and all risk(s) associated with my volunteering. Furthermore, I freely and of my own free will and volition execute this RELEASE AND WAIVER OF RESPONSIBILITY, and agree that neither RESCUE ALLIANCE agents, officers, Board of Directors, volunteers, members, independent contractors, sponsors, employees, managers and/or any other representatives shall incur any responsibility or liability, financial or otherwise, for any injury (including death) and/or damage resulting from negligence and/or any other acts or occurrences, however caused.
I, the above signed, understand that public relations are an important part of volunteering at RESCUE ALLIANCE. On behalf of myself, my heirs, personal representatives, and executors, I allow RESCUE ALLIANCE to use any photographs taken of me or my family for use in public relations efforts and hereby release RESCUE ALLIANCE from any and all liability for said disclosure.
Accordingly, I hereby, for myself, my heirs, my executors and administrators, release and discharge each of the foregoing individuals and entities from all actions, claims, demands, causes of action, suits at law or in equity for any injury or damage suffered or incurred by me due to my volunteer position and/or during my volunteer service with RESCUE ALLIANCE.

I, parent's full name, being the parent or legal guardian of minor's full name, and a current active volunteer of Rescue Alliance Services (herein after referred to as "RESCUE ALLIANCE"), hereby give my consent to allow my child or ward to perform volunteer services for RESCUE ALLIANCE. I fully understand and acknowledge that his/her services are to be performed subject to all the rules and regulations of RESCUE ALLIANCE, that violations thereof shall be cause for immediate dismissal of all services, and that all services performed by my child or ward are strictly voluntary, without pay or compensation or any sort and without liability of any nature on behalf of RESCUE ALLIANCE.
