  • 1. Veterinary Referral 

    1.a. In accordance with the Veterinary Surgeons Act 1966, all veterinary physiotherapist must seek referral from a veterinary surgeon if treating an animal with any pre - existing conditions, injuuries or lameness. All clients must have the referral form completed via their veterinary surgeon prior to appointment. If your animal has not been directly referred to me via your veterinary surgeon, then I will be in contact with your veterinary surgeon prior to your appointment to gain veterinary consent. 

    1.b. Hound & Hoof Veterinary Physiotherapy reserves the right to refuse treatment or refer back to the relevant veterinary surgeon where necessary. 

    2. Cancellation

    2.a. Cancellations are free of charge if made 48 hours before the scheduled appointment time. 

    2.b. Cancellations made less than 24 hours before the scheduled appointment time will incur a full charge of the appointment price. 

    2.c. Cancellations made due to emergency circumstances, less than 24 hours before the scheduled appointment time, may not be charged the full appointment price. However this is at the discretion of Hound & Hoof Veterinary Physiotherapy. 

    3. Payment and Insurance 

    3.a. If clients wish to claim the cost of your animal's physiotherapy treatment through insurance, it is the client's responsibility to check this is covered by their insurance provider. 

    3.b. Invoices and receipts in order to claim treatment through insurance can be provided upon request. 

    3.c. Payment for appointments and treatments must be made up front. BACS is the preferred method of payment, however cash is accepted. 

    3.d. Cost of appointment is determined dependent on species, treatments and travel costs. Payment for appointments and treatments must be paid within 24 hours of the appointment, unless agreed otherwise. 

    3.e. In the event of any difficulty regarding payments, the client must get in touch with Emma of Hound & Hoof Veterinary Physiotherapy as soon as possible. Please note that Hound & Hoof Veterinary Physiotherapy reserve the right to unpaid amounts in any appropriate manner, such as civil proceedings. Any cost that may be incurred through this process will also be added to the outstanding amount, along with standard rates of interest. 

    3.f. Any offers, discounts or deals advertised by Hound & Hoof Veterinary Physiotherapy must be discussed at the time of booking. Any offers, discounts or deals can be withdrawn at any time at the discretion of the business. 

    3.g. Travel charges for mobile clients outside of a certain radius will be calculated and discussed at the time of booking. 

    4. Your obligations 

     4.a. By signing this document, you are confirming that you are the legal owner of the animal undergoing treatment and all information is accurate and up to date.

    4.b. Whilst all treatment is undertaken by a qualified and registered veterinary physiotherapist, you understand that treatment is undertaken at the owner's risk.

    4.c. For mobile clients, you agree to allow the veterinary physiotherapist onto your property and that the area is safe and suitable for treatment to take place. 

    4.d. You take full responsibility in declaring any dangerous behaviours or harmful behaviours which may be displayed by the animal undergoing treatment. It is the owner's responsibilty to ensure these behaviours are mitigated where possible, for example muzzling. 

    4.e. You agree to pay all charges outline in the payment terms. 

    4.f. Owners must inform Hound & Hoof Veterinary Physiotherapy if their animal's conditions worsens during or after the appointment treatment, or if their vet advises againts physiotherapy 

     

    5. Hound & Hoof's obligations to you 

    5.a. Clients will be kept up to date regarding assessment findings, treatments and progress made with their animal. Full written reports will be submitted to your registered veterinary surgeon at appropriate times. 

    5.b. To abide by the National Association of Veterinary Physiotherapist code of conduct. 

    5.c. To ensure professional business insurance and membership to the NAVP is up to date and valid

    5.d. To employ reasonable health and safety measure in regards to the animal being treated and any members of the public or staff within the premises. 

    6. Privacy Policy 

    6.a. All data will be handled in accordance with the Data Protection Act o 1998 and General Data Protection Regulation (GDPR). 

    6.b. Data recieved through referral and information forms is to allow identification of clients and will remain confidential. 

    6.c. Any paper documents are kept within a locked file that only Emma Precious (owner of Hound & Hoof Veterinary Physiotherapy) has access to. 

    6.d. Any electronic records are stored on a password protected computer in a password protected file that Emma Precious (owner of Hound & Hoof Veterinary Physiotherapy) has access to. 

    6.e. All data will be stored for a minimum of 7 years (even after the passing of an animal) for insurance purposes.

    6.f. Information regarding your animal will only be shared with your referring veterinary surgeon, insurance company (if you are claiming through insurance) and any other veterinary professionals caring for your animal, as the owner you will be informed of when and how this information is being shared. 

    7. Videography, Imaging and Social Media 

    7.a. By signing these terms and conditions, you hereby give permission for Hound & Hoof Veterinary Physiotherapy to use images and videos of your animal on various social media platforms for marketing purposes. Any content will be discussed with your prior to posting to obtain further approval before its use. Therefore, you hereby release Hound and Hoof Veterinary Physiotherapy from any and all claims arising from the use of this media. 

     7.b. By signing these terms and conditions, you hereby give permission for Hound & Hoof Veterinary Physiotherapy to obtain images and videos of your animal during static and dynamic assessment for monitoring purposes which may be shared with other veterinary professionals involved in the care of your animal e.g. veterinary surgeon, farrier etc. You, as the owner, will be informed of when and how these videos and images are being shared.
