Individual Health Care Plan Form/Food Allergy Action Plan/Anaphylaxis Emergency Action Plan

If you child has any allergies or a chronic medical condition (such as food allergies, asthma or diabetes) you must fill out the following forms and return them with any related medications BEFORE your child's first day at Woodside Montessori Academy. Your child will not be able to start at Woodside until all appropriate paperwork is in and medications are onsite.

Forms included below:
Food Allergy Action Plan* (required for students with allergies)
Individual Health Care Plan* (required for students with any chronic health condition eg. asthma, diabetes, etc.)
Anaphylaxis Emergency Acation Plan (only required if allergy causes anaphylaxis)
*These forms must be signed by a parent or guardian AND the doctor.

All medication must be submitted in the original box with the perscription label still attached.

Some doctors have these forms on file they can send you directly.
