• AFFORDABLE HOMEOWNERSHIP PROGRAM APPLICATION

    This is an application for Lafayette Habitat for Humanity's Affordable Homeownership Program. This is not an application for a mortgage. Please complete this application truthfully, completely, and accurately and upload ALL required documentation. Failure to submit all required documentation will cause processing delays. All information you include on this application will be maintained in accordance with our privacy policy.

     

    We are pledged to the U.S. policy for the achievement of equal housing opportunity.   • SECTION 1: APPLICANT INFORMATION

    The Applicant would be the owner of the Habitat home and would be responsible for the Habitat mortgage loan. If someone else will be responsible for the mortgage loan along with you, their information will be requested later in the application. Please be sure to answer truthfully, completely, and accurately. *Note - Applicants must be 18 years of age or older. 

    In order to electroncially submit your application, you will need to give your consent to electronically sign your application. Please review the consent to electronic signature information below and check the box if you agree. If you do not agree, you may complete this application, by hand, by picking up a copy of the application at our main office, located at 823 West Congress Street in Lafayette, LA.

    FOR BORROWER SELECTING THE UNMARRIED STATUS

    The lender may use the Unmarried Addendum only when a borrower selected "Unmarried" in Section 1 and the information collected is necessary to determine how state property laws directly or indirectly affecting creditworthiness apply, including ensuring clear title. For example, the lender may use the Unmarried Addendum when the borrower resides in a state that recognizes civil unions, domestic partnerships or registered reciprocal beneficiary relationships or when the property is located in such a state. "State" means any state, the District of Columbia, the Commonwealth of Puerto Rico, or any territory or possession of the United States.

    You do not need to be employed to qualify, but you must have enough regular, reliable income to afford a monthly mortgage payment. Please tell us about your employment by listing all of your employers for the last 2 years.

    The Co-Applicant would be the co-owner of the Habitat home and would have joint responsibility for the Habitat mortgage loan. If you are married and not legally separated, your spouse must be listed as a co-applicant.

    To be considered for a Habitat home, the Primary Applicant and/or Co-Applicant, and their household members, must be willing to complete a certain number of Sweat Equity hours which may include hours spent helping to build the home and the homes of others, attending homeownership and financial literacy classes, and other approved activities.     I understand that by filing an Affordable Homeownership Program application with Lafayette Habitat for Humanity, I am authorizing Lafayette Habitat for Humanity to evaluate my need for a Habitat home, my ability to repay the no-interest loan and other expenses of homeownership, and my willingness to be a full partner in Habitat’s Affordable Homeownership Program. I understand that the evaluation may include, but is not limited to, personal visits, credit checks, rent verification, employment verification, criminal background checks, and sex offender registry checks. I have answered all the questions on this application truthfully. I understand that if I have not answered the questions truthfully, my application may be denied, and that even if I have already been approved to participate in the homeownership program, I may be disqualified from the program. I also understand that my signature on this application gives Lafayette Habitat for Humanity, and its partners, permission to use photographs of me and my household, as well as my bio in appropriate publications.The original or a copy of this application will be retained by Habitat for Humanity even if the application is not approved.By signing this statement I am submitting to such inquiries, allowing the release of my personal information to Lafayette Habitat for Humanity, and certifying that all information submitted on this application is accurate and complete.

Be sure to include your full name as you have it listed on this application on your check or money order. Also indicate "application fee" on the subject/reference line. If you are the co-applicant, please indicate the primary applicant's name as well. 

      For your convenience, we also have a drop box located near the front entrance of our main office should you visit us outside of regular business hours. Please place your payment in a sealed envelope with your name clearly written on the front for processing.
