Formula Zero LLC Release and Waiver
ASSUMPTION OF RISK:
As a condition of Coach or other staff or volunteers (hereafter "Coach") (whose signature appears below) participating in various basketball skills and drills presented by Formula Zero LLC at its Elite Camp at PHHacility, 4600 E. Cotton Gin Loop, Phoenix, AZ 85040 on or about August 12, 2024, Coach agrees that he/she assumes the risk and dangers of accidents, personal and bodily injury (including death) and property damage, and severe social and economic losses. Coach acknowledges that the risks of this event include but are not limited to Coach's own actions, inactions or negligence as well as actions, inactions and negligence of others, including Formula Zero owners or representatives. Coach has considered and evaluated the nature, scope and extent of the risks involved and voluntarily chooses to assume all such risks, both known and unknown and assumes full responsibility for Coach's participation in this event.
RELEASE AND WAIVER OF LIABILITY:
Coach, for himself/herself and on behalf of his/her heirs, estate, insurers, successors and assigns, fully and forever waives, releases and discharges Formula Zero LLC and its owners, affiliates, officers, directors, shareholders, employees, agents, representatives, contractors, successors, assigns, insurers, all event sponsors, advertisers, volunteers, staff, all owners of the premises where the event takes place (collectively the "Released Parties") from any and all injuries (including death), losses, damages, claims, demands, lawsuits, expenses and any other liability of any kind, of or to me, my property, or any other person, directly or indirectly arising out of or in connection with my participation in or attendance at the August 19, 2023 event, even if due to the negligence of one or more of the Released Parties, to the fullest extent permitted by law.
AUTHORIZATION TO USE NAME AND RECORD PARTICIPATION:
The undersigned Coach hereby grants to Formula Zero LLC and its affiliates, successors, assigns and licensees (collectively Formula Zero) permission to film, photograph, video record and otherwise record my image, voice, quotes, name or any other aspect of my attendance at or participation in the August 19, 2023 event and the right, throughout the world, in perpetuity, to register for copyright, to use and to assign and/or license others to use all or a portion of the same in all media and in any manner in connection with the event or otherwise without any additional consideration.
MEDICAL INSURANCE:
Coach represents and warrants that he/she maintains personal insurance for any medical expenses that may be incurred as a result of participation in the event and understands that Formula Zero LLC does not have insurance for such medical expenses that may be incurred from participation in the event.
