To return to swim practice after a suspension, you must complete this Reactivation form.

Submit this form by the 20th of the month PRIOR TO when you wish billing to restart.

Please read this information carefully before proceeding:
- Completing this form will trigger the TAC TITANS Finance Team to restart billing on the date selected.
- Unless returning from Medical Suspension, billing will alwasy re-start on the 1st of the month. No partial month proration allowed for suspensions.
- Jr TITANS, TITANS Prep and Teen TITANS: While suspended your spot may have been filled by someone on a waitlist.
- You must wait to hear back from us to confirm there is room for you to return before coming to the pool.
