WAIVER OF LIABILITY, PRODUCTION RELEASE, and PHOTO IMAGE RELEASE 

I, {participantsName} , hereby understand and acknowledge that this document is a binding legal agreement between myself and Pine Creek Place, its affiliates, officers, directors, shareholders, employees, agents, and Lunch & Recess, hereinafter collectively referred to as "PCP," regarding my participation in event activities, photo shoots, weddings, parties, corporate events, family reunions, dinners, etc (hereinafter referred to as "Pine Creek Activities").

1. RELEASE OF LIABILITY

By signing this document, I voluntarily agree and hereby release and forever discharge any persons, entities, and their respective directors, officers, employees, agents, contractors, partners, shareholders, successors, assigns, parent or subsidiary entities, representatives, members, or affiliates related in any way to PCP from any and all actions, liabilities, damages, losses, costs, expenses, claims, or demands, including but not limited to those based on negligence, gross negligence, and/or product liability, breach of contract, or breach of any statutory or other duty of care owed under applicable laws, that I, my heirs, next of kin, spouse, distributees, guardians, legal representatives, executors, administrators, successors, and assigns now have or may hereafter have for any injury to me or my property, resulting directly or indirectly from my participation in Pine Creek Place activities.

2. IMAGE RELEASE

I hereby grant to PCP, its affiliates, officers, directors, shareholders, employees, and agents, and Lunch & Recess, their heirs, legal representatives, and assigns, the unrestricted right and permission to copyright and use, re-use, publish, and republish video or pictures of me or in which I may be included, intact or in part, composite or distorted in character or form, without restriction from airdate as to changes or transformations, in conjunction with my own or a fictitious name, or reproduction hereof in color or otherwise, made through any and all media now or hereafter known for illustration, art, promotion, advertising, trade, or any other purpose whatsoever. I hereby relinquish any right that I may have to examine or approve the completed product or products or the advertising copy or printed matter that may be used in conjunction therewith or the use to which it may be applied.

3. WAIVER OF LIABILITY

I hereby release, discharge, and agree to save harmless PCP, its affiliates, officers, directors, shareholders, employees, and agents, Lunch & Recess, their heirs, legal representatives, or assigns, and all persons functioning under their permission or authority, or those for whom they are functioning, from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of said video or in any subsequent processing thereof, as well as any publication thereof, including, without limitation, any claims for libel or invasion of privacy.

4. AFFIRMATION

I hereby affirm that I am Eighteen years of age or older. I have carefully read this document, and I understand its contents. I am aware that this document is a release of all liability and a contract enforceable against me (and my heirs, next of kin, distributees, guardians, legal representatives, executors, administrators, successors, and assigns) in a court of law. I have signed this document of my own free will.
