The following programs can use this form to withdraw and end future payments (note refunds will not be issued for payments made):
- Stations 1-7 Lessons
- Adult monthly members
- MS Prep I or II
- Intro to Swim Team, Porpoise Ready and Challenge enrolled in a monthly memberships or 2 payment option
The following programs should use this form to notify team of withdrawal Payments will continue per financial term agreements:
- HS Prep I or II swimmers (financially commited to Sept-Nov enrollment)
- Porpoise Achieve & Performance swimmers (financially committed to Sept- July)
Submission deadlines:
For MONTHLY payment plans, form must be submitted by the 20th of the month to avoid the subsequent month payments.
For SHORT + LONG Course payment plan who want to opt out of the Long Course season (May-July), please submit this form by 4/15 to avoid being charged the second dues installment.
For SCHOOL + SUMMER payment plan who want to opt out of the summer Long Course session (mid June-July), please submit by 5/15.
