Dear Parent/Guardian(s): A great deal of information is presented in a short period of time during the Safe Sitter® course. We want every child to successfully complete the course. Please notify course leaders if your child requires learning accommodations to help them learn the same materials as their peers.

Emergency Medical Permission In the event of a health emergency, I authorize Louisiana Firefighters Foundation to seek emergency care for my child.
Manikin Practice includes practice of rescue skills on CPR manikins. Manikins require strict standards for controlling infection. I agree not to send my child if he/she has a contagious illness including rash. I give permission for my child to practice on the manikins.
Photography, Video, Website, Media and Social Media Release for Participant (or Dependent): I hereby grant full permission to Louisiana Firefighter's Foundation, St. George Fire Department, The Safety Place, their employees, partnering agencies, contractors, and volunteers to use, reuse, reproduce, publish, or republish any photographs, motion pictures, recordings of my child's participation in this program/event through any and all promotion outlets without restriction as to changes or alterations.
