- I understand that submitting this application does not guarantee my admittance to the workshop.
- I have submitted a 60-second video message above.
- I will attend the program in its entirety if selected.
- I will be available to volunteer 8 hours of instruction/classroom time annually to AAMD upon completion of the L&D team AAMD training program.
- I'm responsible for a $350 cancellation fee if I cancel within 30-days of the scheduled training or no-show for the program.
- My supervisor is aware of my application and is supporting me receiving this valuable training in exchange for 8 hours of volunteer instruction to AAMD annually.

- The above candidate will be supported in attending training if selected
- This training is provided FREE of charge by AAMD, but the above candidate will be responsible for volunteering 8 hours of instruction/classroom time annually upon completion of the L&D team AAMD training program.
- The above candidate will have my support in meeting their volunteer instruction commitment
- Should the above candidate fail to follow through with their commitment to the Train the Trainer program portion and subsequently fail to pay the $350 fee, my company will be held responsible.
