I understand that all information and activities associated with canine sports and fitness are to assist in the sharing of information and educational materials. I fully understand that the instructor/representative for Run Jump Play (RJP) is not a veterinarian or canine medical professional in any capacity. The instructor/representative will not diagnose, prescribe or treat any illness, injury, disease or any other physical/mental canine disorder. The instructor/representative will not prescribe any medications, treatment or supplemental remedies associated with any canine disorder or disease. The instructor/representative will not prescribe or recommend canine supplements or dietary changes, without consulting dog owners’ veterinarian. I understand that based on a dog’s current health, some canine conditioning and/or sporting activities may not be recommended and fitness exercises, conditioning and/or sporting may be detrimental to the dog. Run Jump Play will, to the best of their abilities and training design a program or instruct at the level that is right for your dog and assist in making the experience successful. However, results are not guaranteed and/or promised by Run Jump Play or any affiliate instructors or representatives. If my dog is currently participating in a veterinarian recommended treatment; I have or will obtain written approval, with listed restrictions, if any, from the veterinarian for the dog to participate in an canine conditioning/fitness program, sporting program and/or class, prior to beginning any activities with Run Jump Play Owner signed below represents that they are the current registered owner of the dog. The dog is in good health and has not been exposed to communicable diseases within the last 30 days. The dog is current on state mandated vaccinations.

IN CONSIDERATION of being permitted to compete, practice or use equipment and premises in any way in the EVENT or CLASS, EACH OF THE UNDERSIGNED, for himself, his personal representatives, heirs and next of kin:1. HEREBY RELEASES, WAIVES, DISCHARGES AND COVENANTS NOT TO SUE the owners of Run Jump Play K9 Sports Training, Whitney and Charles Lightner, participants, their family or heirs, competitors, exhibitors, instructors and others who give recommendations or instructions, all for the purpose herein referred to as “Releasees”, FROM ALL LIABILITY TO THE UNDERSIGNED, his personal representatives, assigns, heirs, and next of kin FOR ANY AND ALL LOSS OR DAMAGE AND ANY CLAIM OR DEMANDS THEREFOR ON ACCOUNT OF INJURY TO THE PERSON OR PROPERTY OR RESULTING IN DEATH OR INJURY OF THE UNDERSIGNED ARISING OUT OF OR RELATED TO THE EVENT(S), WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE.2. HEREBY AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS the releasees and each of them FROM ANY LOSS, LIABILITY, DAMAGE OR COST they may incur arising out of or related to the UNDERSIGNED INJURY OR DEATH, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE.3. HEREBY ASSUMES FULL RESPONSIBILITY FOR ANY RISK OF BODILY INJURY to Human and Canine participants, spectators and family members, DEATH OR PROPERTY DAMAGE, arising out of or related to the EVENT(S) whether caused by the NEGLIGENCE OR RELEASEES or otherwise.4. HEREBY acknowledges that Canine sports are a physical sport and involves the risk of injury and/or death to the trainer and the canine. Each of THE UNDERSIGNED, also expressly acknowledges that INJURIES RECEIVED MAY BE COMPOUNDED OR INCREASED BY NEGLIGENT RESCUE OPERATIONS OR PROCEDURES OF THE RELEASEES.5. HEREBY agrees that this Release and Waiver of Liability, Assumption of Risk and Indemnity Agreement extends to all acts of negligence by the releasees, INCLUDING NEGLIGENT RESCUE OPERATIONS and is intended to be as broad and inclusive as is permitted by the laws of the State or Province in which the Event(s) is/are conducted and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.6. HEREBY acknowledges that the undersigned has the right to negotiate the terms and conditions of this release agreement but by signing below hereby waives such right.
