Medical Release

I, {primaryParentguardian}, the parent or legal guardian of {studentName}, give permission to P.E. with Coach Jen for any and all medical attention to be administered to my child in the event of an emergency, when the need for treatment is clear, and when all efforts have been made to contact me. By signing below I also assume the financial responsibility of payment for any and all expenses incurred for treatment.

Waiver of Liabiity 

 I, {primary parent/guardian}, parent or legal guardian of {student name} give my full permission for my child to participate in classes and activities at P.E. with Coach Jen. In allowing my child to participate in classes and activities, I fully and completely release, discharge, and hold harmless P.E. with Coach Jen, and its directors, trustees, officers, employees, agents, insurers, instructors, and volunteers (collectively "its agents and employees) from any and all claims, actions, causes of action, and damages of any kind whatsoever, including but not limited to claims for personal injury, death, or property damage, arising out of my child's participation in classes and activities at P.E. with Coach Jen.

I acknowledge and recognize there are inherent risks involved in such activities and I assume the risk of any injury sustained while at P.E. with Coach Jen. I agree to indemnify, defend and hold harmless P.E. with Coach Jen and its agents and employees from any and all claims arising out of my child's participation in any class or activity at P.E. with Coach Jen, even if such claim arises as a result of a negligent act or omission of P.E. with Coach Jen or its agents and employees.

Media Authorization and Release

P.E. with Coach Jen requests permission to photograph and/or record your child while participating in classes and/or activities and for the resulting images and/or recordings to be used for marketing and/or training purposes.

By signing below you herby waive any right to inspect or approve the finished photographs and/or recordings that may be used for such purposes, understand and agree that the use of such images and/or recordings will be without compensation, and that P.E. with Coach Jen will have exclusive rights to the images and/or recordings.

By signing below I agree to the above terms and I hereby release and hold harmeless P.E. with Coach Jen and its agents and employees from any all all claims arising from use of photographs and/or recordings.

Monthly Enrollment: I understand P.E. with Coach Jen offers a monthly class, and if I decide that I will no longer enroll my child for the following month, I will extend the courtesy of giving P.E. with Coach Jen a 30-day notice, so that our spot in the class can be made available to another student.

Communication: The primary means of communication will be through email. If I have any questions about the program or activities I will reach out via email to p.e.withcoachjen@gmail.com or by phone at (559) 213-5761.  

Water: I will send my child with a water bottle.

Holidays/Emergency/Class Cancellations: I am aware that some of the class days may fall on a holiday and that some class days may close due to holidays. In the event of closure due to emergency or other unforeseen circumstances, I am aware that make-up days may not be made available to my child.

Behavior & Conduct: I will do my best, within my abilities as the parent/guardian, to encourage my student to be prepared for class and ready to participate with a positive attitude. I will instruct my child to show respect for P.E. with Coach Jen instructors and other students. I will share with them the expectation that they follow the rules, which includes instructing them to remain with their group and not wander off into other areas. I understand that if there are ongoing behavioral issues or other concerns regarding my child's participation and/or behavior in the class, P.E. with Coach Jen may come to the conclusion that the program is not a good fit for my child, and that they may be dismissed from further instruction. 

Monthly Class Fee: I understand that I am responsible for my child's monthly class fee, which must be received by the 1st of the month and that they may not be allowed to attend class if the payment has not been received and/or confirmed.

Charter School Students: I understand that it is my responsibility to submit the vendor service request for the monthly fee in a timely manner. This usually requires that I submit the request 2 weeks prior to the start of class, in order for P.E. with Coach Jen to receive my approved certificate before class begins. If P.E. with Coach Jen does not receive my approved certificate by the first of the month, I understand that my student may not be allowed to attend class and their spot will be made available to a student on the waitlist.

I also understand that it is my responsibility to be informed of my charter school's specific guidelines regarding vendor service certificates. I understand that if I request services from my charter for use at P.E. with Coach Jen, and I do not utilize the services, that my charter school may not issue me a refund for unused services, and that it is not the financial responsibility of P.E. with Coach Jen to reimburse me for unused services.

Property Damage: I understand that if my child is found to be responsible for any damage to the property where classes are held, I may be financially responsible for the damages.

Refunds: I understand that there are no refunds issued for missed or unused days, including but not limited to my child being ill or absent. If there are closures due to unforeseen events, I understand that we may not have the option for a make-up day.
