DHS OIG Hotline Complaint Form

The mission of the Office of the Inspector General is to detect and deter waste, fraud, abuse, and misconduct in the West Virginia Department of Homeland Security and to promote economy and efficiency in the Department’s operations and agencies. The West Virginia Department of Homeland Security OIG accepts complaints related to the following components:

West Virginia Division of Corrections and Rehabilitation
West Virginia State Police
West Virginia Fire Marshal's Office
West Virginia Division of Emergency Management
West Virginia Fusion Center
West Virginia Parole Board
West Virginia Division of Administrative Services
West Virginia Division of Protective Services

Complaints may be reported anonymously.

Complaint type

Disclose my identity to WV DHS OIG, but request that WV DHS OIG keep my identity confidential with respect to individuals outside WV DHS OIG. It is understood that this selection may hinder WV DHS OIG’s ability to thoroughly review and/or resolve the allegations in the complaint. It is also understood that, despite the request for confidentiality, it may be necessary for WV DHS OIG to disclose my identity to individuals outside WV DHS OIG if WV DHS OIG deems such disclosure (1) necessary during the course of WV DHS OIG’s processing of the allegations in the complaint, or (2) otherwise required by law. WV DHS OIG may also decline the complaint and close it without further action or refer the complaint to another agency with my identifying information redacted. This selection may, therefore, hinder the ability of another agency to thoroughly review and/or resolve the allegations in the complaint.
